Help Clients Build the Bridge Between Treatment and Lasting Recovery
The Raleigh House is seeking a compassionate, organized, and mission-driven Clinical Case Manager to join our interdisciplinary treatment team. This is a full-time, onsite position requiring a consistent five-day-per-week schedule.
Recovery doesn't end when treatment is complete—it continues long after a client leaves our care. As a Clinical Case Manager, you'll play a vital role in helping clients navigate every step of their treatment journey while preparing them for long-term success. You'll advocate for your clients, coordinate essential resources, support treatment planning, and ensure every individual leaves with a personalized plan for continued recovery.

What You'll Do
- Manage an assigned caseload while serving as a trusted advocate throughout each client's treatment journey.
- Coordinate admissions, assessments, treatment planning, and discharge planning to ensure seamless continuity of care.
- Connect clients with community resources, medical providers, aftercare services, legal resources, and other support systems that promote long-term recovery.
- Collaborate closely with therapists, physicians, nurses, Behavioral Health Technicians (BHTs), admissions, utilization review, and families to deliver exceptional client-centered care.
- Facilitate psychoeducational and process groups, as assigned.
- Assist with crisis intervention, behavioral support, and client advocacy while maintaining a safe, structured therapeutic environment.
- Complete timely, accurate documentation while ensuring compliance with regulatory, accreditation, and organizational standards.
- Coordinate warm handoffs to outpatient providers, recovery resources, and continuing care services that support lasting success beyond treatment.
We're looking for someone who is:
- Passionate about helping individuals recover from substance use and co-occurring mental health disorders.
- Highly organized and able to balance multiple priorities in a fast-paced clinical environment.
- A collaborative team player who enjoys working alongside therapists, nurses, physicians, and other behavioral health professionals.
- Skilled at building relationships with clients, families, referral partners, and community resources.
- Compassionate, resourceful, and committed to helping clients overcome barriers to recovery.
- An excellent communicator who values professionalism, accountability, and teamwork.
Qualifications
- Bachelor's degree in Social Work, Psychology, Human Services, Behavioral Health, Counseling, or a related field required.
- Experience in behavioral health, case management, substance use disorder treatment, or a related clinical setting preferred.
- Knowledge of treatment planning, discharge planning, case management, community resources, and continuity of care.
- Familiarity with trauma-informed care, ASAM Criteria, HIPAA, 42 CFR Part 2, and behavioral health best practices.
- Experience working with electronic medical records (EMR) systems.
- Strong organizational, documentation, communication, and critical thinking skills.

Pay: $55,000.00 - $62,000.00 per year
