WHAT YOU BRING
- Credentials: Active MA, LPN, or RN license/certification in the state of Hawaii
- Technical: Computer savvy; intermediate proficiency in MS Suite — Word, Teams, Outlook, and Excel
- Communication: Strong verbal and written communication skills; professional phone etiquette and excellent customer service
- Organization: Excellent attention to detail, strong time management, and ability to prioritize time-sensitive situations with urgency
- Interpersonal: Strong interpersonal skills with the ability to communicate effectively across a variety of people and settings
- Adaptability: Ability to multitask while providing compassionate patient care in a fast-paced environment
- Leadership: Leadership qualities with the ability to work well within a team of healthcare professionals
- Character: Self-motivated, punctual, dependable, trustworthy, honest, and professional with a positive attitude
WHAT YOU’LL DO
- Maintain care coordination dashboards, patient lists, and tracking for CCM/PCM programs
- Track referrals, authorizations, and follow-up appointments across specialty and post-acute services
- Conduct non-clinical patient and family outreach, including welcome calls and appointment reminders
- Coordinate communication between patients, facilities, Care Managers, and providers
- Gather, upload, and organize medical records, wound photos, labs, and imaging reports
- Support scheduling for follow-up visits, interdisciplinary rounds, and provider appointments
- Complete clerical tasks related to Care Management, including outreach, scheduling, and documentation
- Monitor admissions, discharges, and census changes from all care settings
- Prepare patient summaries, reports, and materials for care team meetings
- Escalate clinical concerns promptly to a Care Manager or provider
- Participate in required training, meetings, and compliance activities
