What will you do as a Care Navigator Registered Nurse (RN)
The Care Navigator RN serves as the primary HopeHealth representative within assigned hospitals and physician practices, playing a critical role in advancing the growth, access, and reputation of HopeHealth’s hospice and palliative care services.
This role facilitates seamless transitions to hospice care, including completing admissions and providing General Inpatient (GIP) visits in alignment with the plan of care. The Care Navigator RN also co-leads hospice admission nurses, ensuring high-quality education and patient-centered transitions of care.
As a subject matter expert in all HopeHealth service lines, this position completes goals-of-care conversations, provides education to patients, families, and referral partners, and supports individuals navigating complex serious illness decisions.

What will you do as a Care Navigator Registered Nurse (RN)
- Co-leads and supports hospice admission nurses, promoting best practices in patient education, admissions, and service transitions.
- Conducts goals-of-care conversations, hospice informational visits, and clinical evaluations to determine eligibility and appropriateness for services.
- Completes hospice admissions and initiates care in compliance with all Medicare and state regulatory requirements.
- Provides GIP visits to hospice patients in assigned facilities in accordance with the plan of care.
- Develops individualized plans of care using an interdisciplinary approach, including documentation of Local Coverage Determination (LCD) criteria.
- Performs ongoing assessments to determine continued eligibility for hospice services.
- Supports patient discharges, as appropriate, ensuring continuity and coordination of care.
- Completes accurate, timely documentation in the electronic medical record (EMR) at the point of care.
- Serves as a subject matter expert on all HopeHealth services, including hospice, palliative care, bereavement, caregiver support, dementia services, and community education.
- Clearly communicates service offerings, benefits, and eligibility criteria to patients, families, and referral sources.
- Provides professional and community education to increase awareness and understanding of serious illness care options.
- Cultivates and maintains strong relationships with hospital teams, and physician practices to support referral development and organizational growth.
- Collaborates closely with hospice, palliative care teams, and the Access Center to ensure a seamless, high-quality customer experience.
- Documents territory activity, outcomes, and opportunities in the CRM system in a timely and accurate manner; leverages data to identify growth opportunities.
- Participates in professional and community events to strengthen partnerships and expand market presence.
- Demonstrates flexibility by supporting coverage needs across territories, as required.
- Consistently reflects HopeHealth’s mission, vision, and values, and adheres to all organizational policies, including quality, safety, infection control, and HIPAA compliance.
- Performs other duties as assigned.
What are the qualifications to be a Care Navigator Registered Nurse (RN)
