H-1B Visa NP Jobs
Nurse Practitioners fill a specialty occupation under the H-1B, requiring at least a master's degree in nursing and an active APRN license for the hiring state. Many health systems and federally qualified health centers sponsor H-1B petitions for NP roles, particularly in primary care, psychiatry, and acute care settings where demand consistently outpaces supply.

Confirm the prevailing wage for your specialty
NP prevailing wages differ significantly by specialty and metro area. Run your exact SOC code through the OFLC Wage Search before negotiating your offer so you know the DOL wage level your employer must certify on the LCA.
Target cap-exempt employers in your job search
Hospitals affiliated with universities and federally qualified health centers are often cap-exempt, meaning they can file your H-1B year-round without lottery exposure. Use Migrate Mate to filter NP roles at employers with verified H-1B filing history.
Request premium processing for time-sensitive start dates
If your current OPT or other status expires close to your anticipated start date, ask your employer to file with USCIS premium processing. Approval within 15 business days protects you from a gap in work authorization.
Check O*NET to document specialty occupation eligibility
Adjudicators sometimes issue RFEs for NP roles in non-hospital settings. Pull the O*NET occupation profile showing the master's-level education requirement to give your employer's attorney concrete evidence the role qualifies as a specialty occupation.
Align your offer letter scope with the LCA job duties
Discrepancies between your offer letter and the LCA job duties are a common RFE trigger for NPs switching specialties. Make sure the clinical scope, practice setting, and supervision structure in both documents match exactly before your employer certifies the LCA with DOL.

Find NP JobsNP H-1B Visa: Frequently Asked Questions
Does an NP role qualify as a specialty occupation for H-1B purposes?
Yes. USCIS recognizes Nurse Practitioner as a specialty occupation because the position normally requires at least a master's degree in nursing or a related clinical field. You'll also need a current, state-specific APRN license for the state in which you'll practice. Without both, the petition will not meet the specialty occupation standard, and USCIS is likely to issue a Request for Evidence.
How do health systems typically handle H-1B sponsorship for NPs?
Most sponsoring health systems treat NP H-1B petitions similarly to physician sponsorships. The employer files the Labor Condition Application with DOL, certifying prevailing wage compliance, then files Form I-129 with USCIS. Larger hospital networks often have in-house immigration counsel and established workflows, while smaller outpatient practices may require more lead time. Searching Migrate Mate lets you identify employers with active H-1B filing history for NP roles specifically.
Can I switch NP specialties or practice settings after my H-1B is approved?
A material change in job duties, clinical setting, or supervising structure typically requires your employer to file an amended H-1B petition with USCIS before the change takes effect. Moving from outpatient primary care to an inpatient acute care unit, for example, is likely a material change. Work with your employer's immigration attorney to assess whether an amendment is needed before you transition roles.
What happens to my H-1B status if my APRN license lapses or is under review by the state board?
A lapsed or suspended APRN license creates a direct legal problem for your H-1B status because licensure is a condition of the specialty occupation classification. USCIS can revoke an approved petition if the underlying license requirement is no longer met. Notify your employer's immigration counsel immediately if your license is under any state board review, and do not allow your license to expire during an active petition period.
Are federally qualified health centers a good option for NPs seeking H-1B sponsorship?
Federally qualified health centers are cap-exempt employers under the H-1B statute, which means they can file petitions at any time of year without competing in the annual lottery. This is a meaningful advantage for NPs who need to start employment on a specific date or who missed the current lottery cycle. Many FQHCs actively recruit NPs for underserved communities and have existing H-1B sponsorship infrastructure.
