Verify your ECFMG certification status early
Educational Commission for Foreign Medical Graduates certification is a prerequisite for residency and licensing, and USCIS will scrutinize it during adjudication. Confirm your ECFMG certificate is current and matches the name on your immigration documents before any employer files.
Target federally designated shortage area employers
Health Professional Shortage Areas and Medically Underserved Areas attract health systems actively recruiting family practitioners. These employers often have standing H-1B sponsorship programs and are familiar with the filing timeline, which shortens your time from offer to petition.
Search H-1B sponsoring employers on Migrate Mate
Filter by family medicine and primary care roles to see which health systems have active DOL Labor Condition Application filing history. This surfaces employers already running H-1B pipelines for your specialty, not just those open to it in principle.
Confirm the employer files before April each year
The H-1B cap registration window opens in March. If you're completing residency or fellowship, your offer letter and LCA must be structured so your start date aligns with the October 1 fiscal year start. Missing the window costs you a full year.
Request a prevailing wage level matching your credentials
Use the OFLC Wage Search to look up prevailing wages for family practitioners in your target metro before negotiating. DOL assigns wage levels 1 through 4; board-certified physicians typically warrant Level 3 or 4, which protects you if USCIS issues a wage-related RFE.
Understand cap-exempt employer eligibility
Federally qualified health centers, nonprofit teaching hospitals, and university-affiliated medical groups are often cap-exempt, meaning they can file your H-1B petition outside the lottery at any time of year. Ask explicitly whether the hiring entity qualifies before signing.

Find Family Practitioner JobsFamily Practitioner H-1B Visa: Frequently Asked Questions
Does a Family Practitioner role qualify as an H-1B specialty occupation?
Yes. Family medicine requires a Doctor of Medicine or Osteopathic Medicine degree, making it a clear specialty occupation under USCIS criteria. You'll also need a valid state medical license in the state where you'll practice. USCIS may request documentation showing the employer's minimum degree requirement, so confirm the job posting specifies the MD or DO as required, not preferred.
Can I work as a Family Practitioner while my H-1B is pending?
If you're transitioning from OPT or STEM OPT and your employer filed before your authorization expired, cap-gap rules may extend your work authorization through September 30. If you're already on H-1B and changing employers, you can start with the new health system once they file an H-1B transfer petition, without waiting for approval. Confirm the timeline with your employer's immigration counsel.
Which types of employers sponsor H-1B visas for family medicine roles?
The most active sponsors include large health systems, federally qualified health centers, Veterans Affairs medical centers, and university-affiliated primary care networks. VA and certain nonprofit teaching hospitals are cap-exempt, allowing year-round filing. You can browse employers with verified LCA filing history for family medicine roles on Migrate Mate to identify health systems actively sponsoring before you apply.
How does the H-1B lottery affect my timeline as a family medicine physician?
For cap-subject employers, your petition enters the lottery in April with a potential start date of October 1. If you're finishing residency in June, you'd be without employer-sponsored work authorization for several months unless the employer is cap-exempt or you bridge on OPT. Planning your graduation and offer timing around the lottery window is one of the most common scheduling challenges for international medical graduates.
What documentation does a Family Practitioner need for H-1B sponsorship?
Your employer files Form I-129 supported by your medical degree transcripts, ECFMG certificate if you graduated from a foreign medical school, state medical license, DEA registration if applicable, and a copy of the certified LCA. USCIS typically also requires evidence of board certification or active candidacy. Gather these documents before your offer letter is finalized to avoid delays in the filing timeline.
