H-1B Visa Primary Care Physician Jobs
Primary care physicians are among the most consistently sponsored H-1B visa occupations, driven by persistent shortages in underserved communities. Many positions qualify for cap-exempt filing through hospitals and nonprofit health systems, bypassing the annual lottery entirely. Board certification, state licensure, and an active DEA registration are the credentials employers verify before initiating any H-1B petition.

Verify your ECFMG certification before applying
International medical graduates must hold active ECFMG certification before any U.S. residency program or employer can file an H-1B petition. Confirm your certification status and expiration date early so delays don't stall your petition timeline.
Target cap-exempt employers first
Federally qualified health centers, nonprofit hospitals, and university-affiliated medical practices can file H-1B petitions outside the annual 85,000-slot cap. Focusing your search on these employers eliminates lottery risk and lets you start sooner.
Look up prevailing wages by county before negotiating
Your employer's LCA must certify a wage at or above the DOL prevailing wage for your specific county and practice setting. Use OFLC Wage Search before salary discussions so you know the floor your offer must meet.
Search verified H-1B sponsors on Migrate Mate
Filter primary care openings by employers with documented H-1B LCA filing history on Migrate Mate. This surfaces health systems that have sponsored physicians before, cutting through listings where sponsorship is uncertain or handled case-by-case.
Confirm your specialty aligns with SOC code 29-1215
USCIS maps primary care physicians to SOC code 29-1215 in the H-1B petition. If your role blends urgent care or occupational medicine, confirm with your employer that the correct SOC code is used on the LCA to avoid a specialty occupation RFE.
Secure state licensure before your petition start date
USCIS requires you to hold a valid state medical license by the H-1B start date, not just at petition filing. If your license is pending, ask your employer to request an October 1 start date to give licensure processing enough time.
H-1B Visa Primary Care Physician: Frequently Asked Questions
Do primary care physician H-1B petitions go through the annual lottery?
Not always. Hospitals affiliated with universities, nonprofit health systems, and federally qualified health centers are cap-exempt, meaning they can file H-1B petitions year-round without entering the April lottery. If you're targeting private practices or for-profit clinics, those petitions do count against the annual cap and require lottery selection.
What credentials does an employer verify before filing an H-1B for a primary care physician?
Employers typically confirm ECFMG certification for international medical graduates, a valid state medical license in the state where you'll practice, board certification or active board eligibility, and a DEA registration if prescribing controlled substances. All of these must align with the job duties described in the H-1B petition and LCA before USCIS adjudicates the case.
How do I find primary care employers who have actually sponsored H-1B visas before?
Migrate Mate surfaces employers with verified H-1B LCA filing history filtered by occupation, so you can identify health systems and medical groups that have successfully sponsored primary care physicians rather than relying on job postings that mention sponsorship in vague terms.
Can a J-1 waiver affect my H-1B eligibility as a primary care physician?
Yes. If you completed a J-1 visa clinical exchange program, you're subject to the two-year home-country residency requirement, which blocks H-1B status until it's fulfilled or waived. Conrad 30 waivers, available through state health departments, allow primary care physicians serving underserved areas to waive this requirement and transition directly to H-1B status.
What is the prevailing wage requirement for H-1B primary care physician positions?
Your employer's LCA must certify a wage at or above the DOL prevailing wage for the specific geographic area and practice setting where you'll work. Wage levels vary significantly by county and employer type. You can look up the applicable wage tier using OFLC Wage Search before entering offer negotiations to ensure the proposed salary satisfies DOL requirements.
