Working alongside the Director of Clinical Services, you will play a key role in ensuring exceptional patient care while fostering a positive and supportive work environment.
What You'll Do
As Clinical Manager, you will assist in overseeing the day-to-day operations of our skilled home health division while ensuring compliance with Medicare regulations and agency standards.
Responsibilities include:
- Supervise, mentor, and support RNs, LPNs, Home Health Aides, therapists, and other clinical staff.
- Assist with daily clinical operations and provide leadership support to the Director of Clinical Services.
- Ensure compliance with Medicare Conditions of Participation (CoPs), state and federal regulations, accreditation standards, and agency policies.
- Review clinical documentation for quality, accuracy, timeliness, and regulatory compliance.
- Assist with hiring, interviewing, onboarding, orientation, staff education, coaching, and performance management.
- Coordinate interdisciplinary patient care and maintain effective communication with physicians, referral sources, and community partners.
- Participate in Quality Assurance and Performance Improvement (QAPI) activities, clinical audits, and performance improvement initiatives.
- Conduct case conferences and assist with development and oversight of individualized patient plans of care.
- Assist with staffing, scheduling, and participation in the nursing on-call rotation.
- Ensure documentation is completed accurately and within required timeframes.
- Monitor patient care outcomes and address clinical concerns promptly.
- Ensure staff have the equipment, supplies, and resources needed to provide safe, effective care.
- Assist with survey readiness and maintain compliance with all regulatory requirements.
- Serve as a clinical resource and mentor to all team members.
- Provide skilled nursing visits, admissions, resumptions of care, recertifications, discharges, and other patient visits as needed to support staffing needs and ensure continuity of patient care.
- Serve as Director of Clinical Services in their absence.
Qualifications Required
- Current, unrestricted Ohio Registered Nurse (RN) license.
- Minimum of three (3) years of nursing experience.
- Strong experience in Medicare-certified home health.
- Thorough knowledge of Medicare Conditions of Participation (CoPs), OASIS, and state and federal home health regulations.
- Excellent leadership, communication, organizational, and critical thinking skills.
- Ability to prioritize multiple responsibilities in a fast-paced environment.
- Valid driver's license, reliable transportation, and ability to travel throughout the service area.
- Willingness to perform field visits, including admissions, routine skilled nursing visits, recertifications, resumptions of care, and discharges as needed.
Preferred
- Leadership or supervisory experience in home health.
- Experience with QAPI, clinical audits, and survey preparedness.
- Experience using electronic medical record systems.
