Nurse Practitioner (Primary Care) – Full-Time or Part-Time
Location: Georgia
Schedule: Wed, Thurs, Fri, 8:00 AM – 5:00 PM
Employment Type: Part-Time

About the Role
We are seeking a compassionate and skilled Nurse Practitioner (NP) or Physician Assistant (PA) to join our growing primary care team. The ideal candidate will provide high-quality, patient-centered care to adults ages 18 and above while managing acute and chronic conditions across commercial insurance, Medicare, Medicaid, and Medicare Advantage/value-based care populations.
Experience with eClinicalWorks (eCW) is preferred but not required.
Key Responsibilities
- Provide comprehensive primary care to adult patients (18+) in an outpatient setting.
- Diagnose, treat, and manage both acute and chronic medical conditions.
- Perform detailed patient assessments; order, review, and interpret diagnostic tests.
- Develop individualized, evidence-based treatment plans.
- Identify and close HEDIS and quality-of-care gaps.
- Document all care accurately in the EMR (eClinicalWorks preferred but not required).
- Collaborate with physicians, medical assistants, and care coordination teams.
- Educate patients on disease prevention, medications, and self-management.
- Support population health and value-based care initiatives.
- Promote a positive patient experience and uphold high standards of care.
Qualifications
- Valid and unrestricted Nurse Practitioner (NP/APRN) license or Physician Assistant (PA) license in the State of Georgia.
- National certification as a Nurse Practitioner, such as FNP, AGNP, or equivalent, or Physician Assistant certification, such as PA-C/NCCPA certification.
- 1–2 years of primary care experience preferred but not required.
- Experience working with Medicare, Medicaid, Medicare Advantage, and commercial plans preferred but not required.
- EMR proficiency; eClinicalWorks experience preferred but not required.
- Strong clinical assessment, critical thinking, and communication skills.
- Ability to work collaboratively in a team-based environment.
- Ability to meet credentialing, payer enrollment, and collaborative/supervisory requirements as applicable to role and licensure.
