Nurse Practitioner (Primary Care) – Full-Time or Part-Time

About the Role

We are seeking a compassionate and skilled Nurse Practitioner (NP) or Physician Assistant (PA) to join our growing primary care team. The ideal candidate will provide high-quality, patient-centered care to adults ages 18 and above while managing acute and chronic conditions across commercial insurance, Medicare, Medicaid, and Medicare Advantage/value-based care populations.

Experience with eClinicalWorks (eCW) is preferred but not required.

Key Responsibilities

• Provide comprehensive primary care to adult patients (18+) in an outpatient setting.
• Diagnose, treat, and manage both acute and chronic medical conditions.
• Perform detailed patient assessments; order, review, and interpret diagnostic tests.
• Develop individualized, evidence-based treatment plans.
• Identify and close HEDIS and quality-of-care gaps.
• Document all care accurately in the EMR (eClinicalWorks preferred but not required).
• Collaborate with physicians, medical assistants, and care coordination teams.
• Educate patients on disease prevention, medications, and self-management.
• Support population health and value-based care initiatives.
• Promote a positive patient experience and uphold high standards of care.

Qualifications

• Valid and unrestricted Nurse Practitioner (NP/APRN) license or Physician Assistant (PA) license in the State of Georgia.
• National certification as a Nurse Practitioner, such as FNP, AGNP, or equivalent, or Physician Assistant certification, such as PA-C/NCCPA certification.
• 1–2 years of primary care experience preferred but not required.
• Experience working with Medicare, Medicaid, Medicare Advantage, and commercial plans preferred but not required.
• EMR proficiency; eClinicalWorks experience preferred but not required.
• Strong clinical assessment, critical thinking, and communication skills.
• Ability to work collaboratively in a team-based environment.
• Ability to meet credentialing, payer enrollment, and collaborative/supervisory requirements as applicable to role and licensure.
