Job Description Summary:
This is a full-time, day shift position at McConnell Heart Health Center and some shifts at the Westerville Medical Campus. Master's degree is required.
JOB SUMMARY
Part of a multi-disciplinary clinical team providing direct patient care for individuals enrolled in cardiac, pulmonary and PAD SET Rehab.
MINIMUM QUALIFICATIONS
- Master's degree - Exercise Physiology or related field
- BLS – Basic Life Support
- ACLS – Advanced Life Support
- Years of experience = 2 years
SPECIALIZED KNOWLEDGE
Experience in cardiac, pulmonary and/or SET for PAD Rehab including an internship in cardiopulmonary rehab. The position functions in partnership with a multi-disciplinary team to provide direct clinical management of patients referred to a cardiopulmonary rehab setting. Work requires a high level of problem-solving skills, analysis and interpretation of clinical problems related to cardiac disease, pulmonary disease and/or peripheral arterial disease. Position requires excellent communication skills (verbal and written), active listening skills, problem solving, independent decision making and managing stressful situations. Individuals in this position are required to respond to emergency situations appropriately. Clinical care is documented in the medical record in the form of individualized treatment plans, creation of exercise prescriptions and general health care coordination.

- Experience with Cardiac stress testing
Responsibilities And Duties:
- Completes intake evaluation including clinical assessment, exercise tolerance test (stress test or 6 minute walk test), testing evaluation, documentation and billing
- Develops exercise prescription & progression for patients in all 3 programs
- Case management of patients enrolled in Cardiac, pulmonary and/or PAD SET Rehab. Includes completion and ongoing monitoring of the individualized treatment plan, referral & consultation with dietitian and stress management, smoking cessation as needed, etc. Includes communication with Medical Director and/or referring physician. All documentation completed in CareConnect
- Circulation during exercise classes & CareConnect documentation of exercise, progression, vitals & other pertinent information
- Ability to develop exercise prescriptions to teach and demonstrate strength based exercises, including hand weights, body weight and stretching to progress patients in cardiac, pulmonary and PAD SET rehab.
- Specialized knowledge related to billing for monitored/non monitored cardiac rehab; pulmonary rehab & PAD SET
- Understanding, monitoring and troubleshooting continuous cardiac monitoring
- May provide inpatient Cardiac Rehab teaching in the hospital (Phase I)
