Target licensed home care agencies
Licensed home care agencies are more likely to have formal HR processes and experience hiring OPT workers. Look for agencies accredited by state health departments, as they tend to have the compliance infrastructure needed to support OPT employment paperwork.
Confirm your CIP code alignment
Elderly companion roles must connect to your field of study to qualify for OPT. Health sciences, gerontology, social work, and psychology are common fits. Review your I-20 CIP code with your DSO before applying to confirm the role is authorized.
Ask about STEM OPT extension eligibility early
If your degree is in a STEM-eligible field like health informatics or psychology, ask whether the employer is enrolled in E-Verify. STEM OPT adds 24 months, giving you significantly more time to build experience and pursue sponsorship.
Highlight certifications that strengthen your application
CPR certification, First Aid training, and any dementia care coursework signal readiness to employers unfamiliar with OPT. Certifications reduce perceived hiring risk and make it easier for smaller agencies to justify the additional documentation steps required.
Be direct about your OPT timeline when interviewing
Employers in elder care often value reliability and long-term commitment. Proactively explain your OPT end date and mention H-1B visa or sponsorship options so employers understand the path forward before making a hiring decision.
Look for nonprofit and government-affiliated programs
Area Agencies on Aging and Medicaid-funded companion programs sometimes operate under structured hiring policies that accommodate work authorization. These organizations often have dedicated HR staff more familiar with I-9 and OPT documentation requirements.
Elderly Companion OPT: Frequently Asked Questions
Does an elderly companion role qualify for OPT work authorization?
Yes, if the role is directly related to your field of study. Students in gerontology, social work, health sciences, nursing, or psychology programs are the strongest fit. You'll need your DSO to confirm the connection between the job duties and your CIP code before you begin work, as unauthorized employment can jeopardize your F-1 status.
Can I work part-time as an elderly companion while on pre-completion OPT?
Yes, pre-completion OPT allows part-time work of up to 20 hours per week while school is in session. Many elderly companion positions offer flexible scheduling, which makes them compatible with pre-completion OPT. Once you've graduated and are on post-completion OPT, full-time work is permitted and most agencies will hire you in that capacity.
Where can I find elderly companion jobs that are open to OPT students?
Migrate Mate is the recommended platform for F-1 OPT students searching for roles like elderly companion that are open to candidates with work authorization. Searching through a general job board often means filtering through listings where OPT authorization is unclear or unwelcome. Migrate Mate surfaces employers already open to sponsoring or hiring OPT workers.
Will an elderly companion employer need to do anything special to hire me on OPT?
The employer needs to complete standard I-9 verification using your EAD card as proof of employment authorization. No petition or USCIS filing is required on their end during the OPT period itself. If you're pursuing a STEM OPT extension, the employer must be enrolled in E-Verify and sign a training plan, which is more involved but still straightforward for most agencies.
Can an elderly companion role lead to H-1B sponsorship?
It depends on the employer and how the role is classified. Companion roles are generally not classified as specialty occupations under H-1B standards, which require a position that normally requires at least a bachelor's degree in a specific field. However, roles within elder care organizations that involve care coordination, case management, or health program oversight can qualify if structured correctly. Discuss this with your employer and an immigration attorney early.
