How to Get Visa Sponsorship as a Field Nurse Case Manager
Confirm your RN license transfers across state lines
Many Field Nurse Case Manager roles require a license in the state where patients live, not just where you're based. Check whether the employer operates in a Nurse Licensure Compact state before applying, so your license covers the territory.
Target managed care organizations and insurance carriers
Large managed care organizations and health insurers hire field nurses at scale and have established HR processes for work authorization. They're more likely to understand OPT than smaller home health agencies, which often lack immigration experience entirely.
Document field hours carefully for your DSO
Field roles involve dispersed schedules across patient sites, which can complicate full-time hour verification. Keep a detailed log of visit records and employer timesheets to demonstrate consistent full-time employment to your DSO throughout your OPT period.
Clarify whether the role qualifies under your degree field
Your OPT authorization must align with your degree's field of study. A nursing or healthcare administration degree clearly supports this role, but confirm the job description emphasizes clinical case management, not purely administrative functions, to avoid DSO authorization issues.
Raise OPT after demonstrating your clinical value
Bring up OPT authorization after a hiring manager has engaged with your clinical background and qualifications. Framing it as straightforward paperwork requiring no upfront cost to the employer, early in the conversation, reduces the chance they disengage before understanding the process.

Get Access To All JobsFrequently Asked Questions
Can I work as a Field Nurse Case Manager on F-1 OPT?
Yes, if you hold an active RN license and your OPT is authorized in a healthcare or nursing-related field of study. Field Nurse Case Manager roles involve direct clinical judgment and care coordination, which aligns clearly with nursing degree programs. Confirm with your DSO that the job description matches your authorized field before accepting an offer.
Do Field Nurse Case Manager employers typically sponsor OPT students?
Large managed care organizations, national insurers, and hospital health systems are the most OPT-friendly employers in this space. Smaller home health agencies may be unfamiliar with work authorization requirements. Searching on Migrate Mate lets you filter for employers with a history of sponsoring international nurses, saving significant time in your job search.
Does this role qualify for the 24-month STEM OPT extension?
It depends on your degree and your employer. If you graduated from a STEM-designated program such as Health Informatics, Biomedical Engineering, or a qualifying clinical science, and your employer is enrolled in E-Verify, you may qualify for the STEM extension. A standard nursing degree (BSN) is not on the STEM designated degree program list, so check your specific CIP code with your DSO.
How do I handle OPT reporting requirements in a field-based nursing role?
Field roles present a specific reporting challenge because you may not have a fixed office address. You're still required to report your employer's primary business address to your DSO within 10 days of starting work. If your assignment area changes significantly or you shift to a different employer entity, update your records promptly to stay in valid OPT status.
What happens to my OPT if I lose my nursing job in this field?
You enter the standard 60-day unemployment grace period, during which you must either secure new qualifying employment or depart the U.S. Field nurse roles are in high demand across managed care, workers' compensation, and disability management sectors, so re-employment is often achievable within that window. Use that time to actively search and document every application made during the gap period.
