Join our team as a day shift, full time, RN Care Manager - Cardiology, at INTEGRIS Health Baptist Medical Center, in Oklahoma City, OK.

ROLE AND RESPONSIBILITIES
The CV Case Manager responsibilities include, but are not limited to, the following:
- Assesses the patient continuously for medical necessity, appropriate level of care, financial situation and discharge barriers
- Responsible for completing physical assessments upon admission
- Initiates discharge planning and will collaboratively work with the interdisciplinary team to identify and coordinate needed services and resources
- Provides education to patient's family and staff as required
- Audits data abstraction for accuracy and provides concurrent feedback to physicians regarding quality measures
The CV Case Manager reports to the Lead CV Case Manager. This position may have additional or varied physical demand and/or respiratory fit test requirements. Please consult the Physical Demands Project SharePoint site or contact Risk Management/Employee Health for additional information.
- Potential for exposure to infectious diseases, bloodborne pathogens and other infectious materials as defined by OSHA. Must follow standard precautions.
BASIC QUALIFICATIONS
License/Certifications:
- RN (Registered Nurse) Current licensure as a Registered Nurse (RN) in the State of Oklahoma or current multistate license from a Nurse Licensure Compact (eNLC) member state
- BLS (Basic Life Support) Issued by American Red Cross or American Heart Association within 30 days of hire
Must be able to communicate effectively in English (verbal/written).
PREFERRED QUALIFICATIONS
Experience:
- 5 years clinical experience in an acute care environment OR 3 years experience with a Bachelors degree in Nursing
- Cardiology experience
