Job Summary:
Provide patient-centered physical therapy to help individuals improve strength, restore movement, and progress toward functional goals. Assess patient needs, develop and adjust treatment plans, monitor outcomes, and coordinate care across appropriate settings, including inpatient, outpatient, and home health services.

Responsibilities:

• Evaluate patients and develop individualized care plans using knowledge of anatomy and physiology.
• Deliver rehabilitation services, including orthopedic, neurological, geriatric, and pediatric care as appropriate to clinical experience.
• Provide therapeutic interventions such as exercise and trigger point therapy, and monitor patient responses.
• Document evaluations, treatments, and progress in electronic health record or electronic medical record systems in accordance with clinical documentation standards.
• Coordinate care with healthcare professionals, communicate with patients and families, and contribute to discharge planning.
• Support continuity of care through clear treatment plans and timely clinical documentation.

Qualifications:

• Hold a current, unrestricted physical therapist license issued by the applicable jurisdiction where services are provided.
• Meet the jurisdiction’s education, examination, and credentialing requirements for physical therapist licensure.
• Maintain any required professional credentials and comply with applicable laws, regulations, and clinical practice standards.
• Demonstrate knowledge of anatomy, physiology, medical terminology, patient care, rehabilitation, and treatment plan development.
• Communicate effectively with patients, families, and healthcare professionals.
• Use electronic health record or electronic medical record systems and follow clinical documentation standards.
• Experience in acute, inpatient, hospital, outpatient, rehabilitation, clinic, or home and community care settings is preferred.
• Experience with geriatric patients and familiarity with pediatric, neurological, orthopedic, or home health care is preferred.

Pay: $65.00 - $70.00 per hour
