Compensation: Starting at $45 per hour. The final hourly rate will be based on verified experience in Medicare-certified home health, OASIS review, QAPI, clinical compliance, and survey preparation.

Position Overview

Dignified Nursing Home Health Care Agency, LLC is seeking an experienced Registered Nurse to assist with clinical quality assurance, OASIS review, QAPI, Medicare compliance, and survey readiness.

This is not an entry-level position. Applicants must have recent, verifiable experience working with a Medicare-certified skilled home health agency. Experience limited to non-medical home care, assisted living, or private-duty caregiving does not meet the requirements for this position.

Primary Responsibilities

• Review OASIS-E2 SOC, ROC, Recertification, Transfer, and Discharge assessments for accuracy, completeness, consistency, and timeliness.
• Review clinical documentation, plans of care, orders, visit notes, and care-coordination records.
• Maintain and update the agency’s QAPI program, quality indicators, meeting minutes, corrective actions, and performance-improvement projects.
• Assist with Medicare, Texas HCSSA, and accreditation survey readiness.
• Conduct internal audits and mock surveys.
• Monitor clinical-record compliance and assist with correcting documentation deficiencies.
• Educate clinical staff regarding OASIS, documentation, Medicare requirements, and agency policies.
• Assist with infection control, emergency preparedness, personnel-file compliance, and other regulatory requirements.
• Work closely with the Administrator and Director of Nursing to maintain continuous compliance.

Required Qualifications

• Current and unrestricted Texas RN license.
• Minimum of three years of recent Medicare-certified home health experience.
• Hands-on OASIS experience, including SOC, ROC, Recertification, Transfer, and Discharge assessments.
• Demonstrated experience with QAPI and clinical quality assurance.
• Direct experience preparing a home health agency for a Texas HHSC, CMS, ACHC, CHAP, or Joint Commission survey.
• Strong knowledge of Medicare Conditions of Participation and Texas HCSSA requirements.
• Excellent organizational, communication, and clinical-documentation skills.
• Ability to provide verifiable professional references.
• Must be available to work on-site at our Houston office.

Preferred Qualifications

• BSN or MSN.
• Previous DON, Alternate DON, Clinical Manager, or Quality Assurance experience.
• Experience with Kinnser/WellSky.
• Experience developing corrective-action plans and performance-improvement projects.
• OASIS certification or formal OASIS training.
• Familiarity with VA Community Care requirements and HSRM.
