Hometown Care is a locally owned and operated home health agency committed to providing compassionate, high-quality care throughout Northeast Ohio. We are seeking an experienced Registered Nurse Clinical Manager to help lead our skilled home health team and support our continued growth.

Working alongside the Director of Clinical Services, you will play a key role in ensuring exceptional patient care while fostering a positive and supportive work environment.

What You'll Do

As Clinical Manager, you will assist in overseeing the day-to-day operations of our skilled home health division while ensuring compliance with Medicare regulations and agency standards.

Responsibilities include:

• Supervise, mentor, and support RNs, LPNs, Home Health Aides, therapists, and other clinical staff.
• Assist with daily clinical operations and provide leadership support to the Director of Clinical Services.
• Ensure compliance with Medicare Conditions of Participation (CoPs), state and federal regulations, accreditation standards, and agency policies.
• Review clinical documentation for quality, accuracy, timeliness, and regulatory compliance.
• Assist with hiring, interviewing, onboarding, orientation, staff education, coaching, and performance management.
• Coordinate interdisciplinary patient care and maintain effective communication with physicians, referral sources, and community partners.
• Participate in Quality Assurance and Performance Improvement (QAPI) activities, clinical audits, and performance improvement initiatives.
• Conduct case conferences and assist with development and oversight of individualized patient plans of care.
• Assist with staffing, scheduling, and participation in the nursing on-call rotation.
• Ensure documentation is completed accurately and within required timeframes.
• Monitor patient care outcomes and address clinical concerns promptly.
• Ensure staff have the equipment, supplies, and resources needed to provide safe, effective care.
• Assist with survey readiness and maintain compliance with all regulatory requirements.
• Serve as a clinical resource and mentor to all team members.
• Provide skilled nursing visits, admissions, resumptions of care, recertifications, discharges, and other patient visits as needed to support staffing needs and ensure continuity of patient care.
• Serve as Director of Clinical Services in their absence.

Qualifications Required

• Current, unrestricted Ohio Registered Nurse (RN) license.
• Minimum of three (3) years of nursing experience.
• Strong experience in Medicare-certified home health.
• Thorough knowledge of Medicare Conditions of Participation (CoPs), OASIS, and state and federal home health regulations.
• Excellent leadership, communication, organizational, and critical thinking skills.
• Ability to prioritize multiple responsibilities in a fast-paced environment.
• Valid driver's license, reliable transportation, and ability to travel throughout the service area.
• Willingness to perform field visits, including admissions, routine skilled nursing visits, recertifications, resumptions of care, and discharges as needed.

Preferred

• Leadership or supervisory experience in home health.
• Experience with QAPI, clinical audits, and survey preparedness.
• Experience using electronic medical record systems.
