Expressions Unlimited is seeking an experienced and organized Assistant Service Coordinator to support our service-coordination department. This position helps ensure that individuals receiving services through New Mexico’s Developmental Disabilities (DD) Waiver receive safe, consistent, and person-centered care.
The ideal candidate will have healthcare or developmental-disability experience and be familiar with Medicaid Waiver services, documentation, and compliance requirements.
Responsibilities
- Assist the Service Coordinator with daily service-coordination and administrative responsibilities.
- Monitor individuals’ healthcare needs, appointments, follow-up recommendations, and documentation.
- Coordinate medical, dental, therapy, and behavioral-health appointments.
- Communicate with guardians, case managers, healthcare providers, pharmacies, and interdisciplinary team members.
- Maintain accurate medical records, physician orders, appointment documentation, and health information.
- Review staff documentation for accuracy, completeness, and DD Waiver compliance.
- Help ensure Individual Service Plans, healthcare plans, assessments, and annual documents remain current.
- Monitor medication records and report missing documentation, discrepancies, or health concerns.
- Assist with General Event Reports, incident follow-ups, and corrective actions.
- Help prepare records for audits, state surveys, and monitoring visits.
- Track appointments, assessments, staff training, certifications, and renewal deadlines.
- Guide Direct Support Professionals on documentation and individuals’ support needs.
- Participate in IDT meetings, healthcare meetings, and staff meetings as needed.
- Maintain confidentiality and follow HIPAA, DDSD, DD Waiver, and company requirements.
- Promptly report health, safety, compliance, or service-delivery concerns to management.
- Complete additional service-coordination duties as assigned.
Qualifications
- Healthcare, developmental-disability, case-management, or service-coordination experience is required.
- Familiarity with New Mexico’s DD Waiver or another Medicaid Waiver program is required.
- Experience supporting individuals with intellectual or developmental disabilities is strongly preferred.
- Knowledge of person-centered services, ISP implementation, healthcare documentation, and compliance requirements.
- Experience using Therap or a similar electronic documentation system is preferred.
- Strong communication, organization, time-management, and follow-up skills.
- Ability to manage multiple deadlines and maintain accurate, confidential records.
- Comfortable communicating with healthcare providers, guardians, case managers, and direct-care staff.
- Proficiency with email, Microsoft Office, Google Workspace, and electronic recordkeeping.
- Must pass all required background checks and meet New Mexico DDSD employment requirements.
- A valid driver’s license and reliable transportation may be required.
Education
- High school diploma or equivalent required.
- An associate’s or bachelor’s degree in healthcare administration, social work, human services, nursing, psychology, or a related field is preferred.
- Relevant healthcare and DD Waiver experience may be accepted instead of a degree.
Pay: $35,000.00 - $50,000.00 per year
