JOB TITLE: Retention Specialist
DEPARTMENT: Rapid Re-Housing (New Start Program)
FLSA: Non-Exempt
REPORTS TO: Housing & Retention Services Coordinator
LOCATION: Yonkers, NY
SALARY: $40,000 - $45,000.
SUMMARY
The Retention Case Manager provides home-based services and support to disabled, non-disabled, employed, and employable families and single adults in permanent housing.
DUTIES & RESPONSIBILITIES:

Responsible for conducting clients’ intakes and obtaining needed documentation to ensure clients' eligibility for the referred department/program. Maintains accurate and up-to-date progress notes in HMIS.

Able to travel within Westchester County.

Works closely with the Department of Social Services and the Westhab Housing Department staff to achieve clients' permanent housing goals as well as with shelter and transitional shelter staff.

Attends weekly cross-department meetings and others as required. Creates and maintains ongoing communication, both verbal and written, between all parties involved (shelters, DSS, housing department, clients, landlords, and collaborating agencies, etc.)

Identifies clients' barriers and develops a service plan with clients to address the identified barriers. Responsible for referring clients to mental health and substance abuse services if needed.

Promotes housing stability and individual well-being during program enrollment. Provides six-month retention services to clients who have exited the program.

Responsible for compiling the initial/renewal lease program/housing documents for submission. Responsible for completing program documentation, reports, assessments, HQS Inspections, and Rent Reasonableness in a timely manner.

Ensures that children are enrolled and attending school. Responsible for reporting any child abuse or neglect.

Requires the completion of various programmatic time sheets and activity logs along with maintaining an updated working calendar.

Responsible for scheduling one client’s home visit as often as needed. Identifies program needs and makes recommendations.

Other duties as assigned.
EXPERIENCE, EDUCATION AND SPECIAL REQUIREMENTS:
Bachelor Degree or Associate Degree required in social work or related field preferred, with two years’ experience in direct client services. Fluency in Spanish is highly desirable. Detail-oriented to complete requirements of files and contract compliance packets. Excellent written and communication skills. Ability to adapt to the client’s and staff needs as needed. Possess strong organizational skills with the ability to meet a demanding workload. Knowledge of community services, conflict and resolution, and strengths-based case management. Some evenings and weekends as needed. Must have own transportation and valid driver’s license.
