Job Description
Overview

This position has an anticipated start of March 1, 2027.
Location: Fort Wayne Hospital in Fort Wayne, IN.
Shift: Full Time M-F with rotating weekend coverage; Positions will support inpatient only - intensive care units, progressive care units, and medical-surgical patients.
Requirements
- Requires a Doctoral Degree from an accredited academic program in physical therapy or a Bachelor’s/Master’s Degree in physical therapy prior to 2016. If foreign trained, graduation from a physical therapy education program outside the US with educationally equivalent coursework accepted by Indiana PLA.
- Requires current Physical Therapist license in the State of Indiana.
- Requires Basic Life Support certification through the AHA.
- Advanced life support certifications may be required per unit/department specialty according to patient care policies.
Roles and Responsibilities:
Provides skilled physical therapy services to patients across the lifespan whose illness, injury, or condition impacts functional mobility, movement, and physical independence. Evaluates physical performance and develops individualized plans of care that address musculoskeletal, neuromuscular, cardiopulmonary, balance, strength, endurance, pain, and environmental factors affecting function. Implements evidence-based interventions to improve mobility, physical function, safety, and participation in daily activities. Collaborates with patients, caregivers, and the interdisciplinary care team to optimize outcomes, quality of life, and safe transitions of care. Supervises physical therapist assistants, students, and support personnel in accordance with state regulations and organizational policy. Participates in activities that support patient care, quality improvement, education, safety, and department operations while advancing IU Health's mission, vision, and values.

Clinical Practice - Performs physical therapy evaluations, assessments, treatment planning, treatment interventions, reassessments, and re-evaluations in accordance with the APTA Guide to Physical Therapist Practice. Provides direct patient care to address functional limitations affecting mobility, movement, safety, and physical performance. Develops individualized plans of care with measurable goals, provides patient and caregiver education, collaborates with patients, caregivers, and the healthcare team, and identifies discharge planning needs. Completes accurate and timely documentation, coding, and charge capture in compliance with department, regulatory, and payer requirements. Initiates referrals when patient needs fall outside the scope of physical therapy practice.
Ethical Practice - Maintains patient confidentiality and HIPAA compliance while demonstrating integrity, professionalism, and ethical decision-making. Builds trust and treats others with respect.
Training/Patient Education - Provides education to patients and families based on individual needs and evaluates understanding and effectiveness. Supports the development of others through mentorship, precepting, and participation in educational and professional development activities.
Safety Awareness - Uses equipment appropriately and follows established safety practices to maintain a safe environment for patients, coworkers, and visitors.
Relationship Building and Customer Care - Builds positive relationships with patients, caregivers, colleagues, and community partners. Demonstrates excellent customer service through respect, responsiveness, and a commitment to meeting customer needs. Represents IU Health and Rehabilitation Services in a professional manner.
Oral/Written Communication - Communicates clearly, professionally, and effectively in verbal and written formats. Actively listens, shares information appropriately, and follows instructions to support quality patient care and team collaboration.
