BAYADA Home Health Care has an immediate opening for a Client Service Coordinator in our West Chester, PA Habilitation Home Care office.

Responsibilities
- Coordinate day to day services and schedules for individuals with I/DD
- Focus on managing coordination of client services and emergent scheduling issues
- Manage your Client Services Coordinator caseload while proactively growing it
- Build lasting relationships with clients, referral sources, payors and community organizations
- Develop strong, communicative relationships with the team
- Maintain effective fiscal management of your caseload by monitoring metrics and client authorizations
- Client Services Coordinators will partner with Clinical Managers to provide supervision and support to Habilitation Technicians
- In collaboration with the Clinical Manager, Habilitation Manager, Direct Support Professionals (DSP) and other teams to act as an advocate for clients in the Habilitation program
- Collaborate with the client, family, Case Managers, DSPs and other teams to implement and evaluate client scheduling needs
- Maintain ongoing communication with Case Managers/Care Coordinators and other stakeholders
QUALIFICATIONS
Bachelor degree is required
At least 1 year of experience with the I/DD population (Preferred)
Prior medical office or home care experience preferred
Prior supervisory experience a plus
Demonstrated record of successfully taking on increased responsibility (goal achievement)
Ambition to grow and advance beyond current position
Strong computer skills required (electronic medical record)
Excellent communication and interpersonal skills
