ROLE AND RESPONSIBILITIES
How you'll make an impact in this role:
- Orchestrate the full interdisciplinary plan of care from admission through discharge to ensure a seamless patient experience.
- Design personalized care strategies that integrate clinical requirements with patient and family values and preferences.
- Monitor patient progress and oversee medical documentation to ensure treatment options and services align with diagnostic needs.
- Advocate for essential resources and proactively remove barriers to ensure the effective delivery of the health plan.
- Maintain continuous collaboration with supervisors and providers to optimize care implementation and post-discharge success.
BASIC QUALIFICATIONS
What minimum qualifications you'll need:
Licensure / Certification / Registration:
- Registered Nurse credentialed from the Florida Board of Nursing obtained prior to hire date or job transfer date required.
- BLS Provider preferred. American Heart Association or American Red Cross accepted.
Education:
- Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire required.
