Position Summary:
Westhab is seeking a dedicated and compassionate Case Manager to join our Adult Housing & Services team in Yonkers. The Case Manager is responsible for assisting clients with applying for Social Security benefits, state disability or retirement benefits, and other entitlements. This role works closely with internal staff and external agencies to ensure clients gain access to a continuum of care that addresses healthcare, housing, employment, disability/retirement benefits, transportation, and other essential support services. The ideal candidate will have strong case management experience, excellent communication skills, and a commitment to serving vulnerable populations with dignity and professionalism.
Duties & Responsibilities
- Complete HMIS intakes, check-ins, annual assessments, and exit assessments as required.
- Conduct CHAT assessments and collect required supporting documentation, including proof of income, identification cards, birth certificates, Social Security cards, and benefit cards.
- Assist clients in accessing essential services, including: SSI/SSD benefits, Medicaid/Medicare and SNAP benefits, housing opportunities, employment and job training services, educational services, substance abuse and mental health treatment, medical care and transportation coordination.
- Meet with clients bi-weekly and/or monthly to develop a rapport and to review the care plan. Provide guidance and coaching to clients on personal hygiene, self-care practices, and healthy daily living skills as part of individualized service planning.
- Provide ongoing support services to individuals with physical health, mental health, and/or substance use challenges.
- Advocate on behalf of clients with medical providers, social service agencies, homeless court, disability benefit offices, and other organizations.
- Build and maintain strong relationships with community partners, referral agencies, and service providers. Works closely with the building’s maintenance workers, the Security Staff and Security Supervisor and other site staff.
- Attend virtual and in-person meetings, trainings, and collaborative case conferences as needed.
- Maintain accurate and timely case notes, reports, and required program documentation to ensure compliance and measure program effectiveness.
- Prepare and submit daily, weekly, and monthly reports as assigned.
- Responsible for completing incidents reports, contact the crisis team, EMS or the Yonkers Police if needed.
- Demonstrate the ability to de-escalate crisis situations effectively using calm, trauma-informed, and client-centered intervention techniques to ensure client safety and stability.
- Report barriers to service access to supervisors and collaborate on solutions.
- Ability to assist with light physical tasks, as needed, in collaboration with maintenance staff to help maintain safe, sanitary, and habitable living environments for clients. Ability to inspect and review clients’ personal belongings, when necessary and in accordance with agency policy, to ensure compliance with program rules, maintain safety standards, and address health or sanitation concerns while respecting client dignity and confidentiality.
- Transport clients to appointments and offsite destinations using Westhab vehicles, as needed.
- Maintain confidentiality of client and agency information at all times.
- Work closely with the Program Coordinator and Director to enhance service delivery and client outcomes.
Experience, Education and Special Requirements:
SOAR Certification required, or ability to complete certification within six months of hire as long as training is available. Associate’s or Bachelor’s degree in Social Work or a related field preferred. Minimum of two years of experience in direct client services required. Strong written, verbal, organizational, and computer skills required. Valid driver’s license and reliable personal transportation required. Ability to drive Westhab vehicles as needed. Able to build partnerships with local service providers, businesses, and law enforcement agencies. Flexibility to work evenings, weekends, and adjusted schedules when program needs require.
