How you'll make an impact in this role:
- Investigate, assess, and plan interventions to help patients cope with social, emotional, economic and environmental problems.
- Interview patients, assesses priorities and documents case activity.
- Assist clients/families with obtaining community assistance by referral to proper resources.
- Develop and maintain working relationships with community health, welfare and social agencies and seeks creative means to assist the client with his/her needs.
- Support management of concurrent review, discharge planning processes, social services, home care and other post discharge needs.
What minimum qualifications you'll need:
Licensure / Certification / Registration: Social Worker credentialed from the Florida Board of Clinical Social Work, Marriage and Family Therapy and Mental Health Counseling preferred.
Education: High school diploma/GED with 2 years of experience, or Associate's degree, or Bachelor's degree required.
Work Experience: 3 years of experience preferred.
