Position Summary:
Join our compassionate team at Trinity Health Grand Rapids Hospital, where your skills in clinical social work will make a critical impact during some of the most vulnerable moments in our patients’ lives. In this fast-paced environment, you will conduct psychosocial assessments, provide brief solution-focused counseling, and develop collaborative treatment goals. Your work will help patients navigate crisis situations, overcome barriers, and access needed community resources to ensure safe and effective care transitions.
What You'll Do:
- Provide social work services during the 8:00 AM – 4:30 PM shift in the inpatient hospital on the weekends only
- Conduct thorough psychosocial assessments for patients and families in crisis.
- Develop individualized treatment and discharge plans that address emotional, social, and practical needs.
- Deliver brief counseling and crisis intervention.
- Coordinate with community agencies and internal resources to support safe care transitions.
- Serve as a patient advocate and liaison between care teams and families.
- Document assessments, plans, interventions, and outcomes in a timely and accurate manner.
- Collaborate with the interdisciplinary care team to support holistic patient care.
What You'll Bring:
- Education: Master’s Degree in Social Work or a related field from an accredited institution.
- Licensure: Eligible for licensure as a Limited Licensed Master Social Worker (LLMSW) or Licensed Master Social Worker (LMSW) by the Michigan Board of Social Work.
- Experience: Minimum of 2 years of social work experience in a healthcare setting; experience in emergency, behavioral health crisis, or acute hospital preferred.
