Responsibilities
- Deliver empathetic and compassionate care to patients in their homes or facilities.
- Assist patients with activities of daily living, pain management, and symptom control.
- Provide emotional support and companionship to patients and their families.
- Coordinate care with the interdisciplinary hospice team.
- Maintain accurate patient records and documentation.
- Ensure a peaceful and comfortable environment for patients.
- Act as an advocate for patients and their families.
QUALIFICATIONS
- Valid Certified Nursing Assistant (CNA) certification.
- Valid Driver's license and state required Auto Insurance.
- A deep sense of empathy and compassion for patients and their families.
- Excellent communication and listening skills.
- Ability to provide physical and emotional comfort to those in need.
- Collaboration skills to work effectively with the hospice care team.
- Understanding of end-of-life care principles (experience in hospice is a plus).
