The Role: As our Community Liaison, you will be the face of Comfort Keepers. Your primary objective is to drive revenue by establishing trusted referral partnerships with clinical organizations (Hospitals, Skilled Nursing Facilities, and Rehab Centers). You will navigate complex healthcare networks, connect directly with clinical decision-makers (Directors of Nursing, Discharge Planners, Case Managers), and position our agency as the premier solution for safe patient transitions.
Key Responsibilities:
- Proactive Territory Management: Complete a minimum of 30 strategic, in-person field visits per week to local healthcare facilities.
- Targeted Clinical Outreach: Focus 75% of your field time on high-volume medical facilities with active patient discharge pipelines.
- Strategic B2B Meetings: Secure and execute at least 4 pre-scheduled, face-to-face accounts meetings with key medical decision-makers weekly.
- Educational Presentations: Host professional lunch-and-learns and in-services to educate healthcare staff, social workers, and families on the value of our care models.
- Pipeline Tracking: Maintain meticulous records of daily activities, follow-ups, and referral conversions utilizing our CRM platform.
