BAYADA Home Health Care is currently seeking an experienced Occupational Therapist, OT, for a Full time opportunity performing Outpatient home health visits for our Vermont Visits office, located in Rutland, Vermont. This office services adult and geriatric patients on a per visit basis in homes throughout Washington and Orange County, Vermont.
As a home care Occupational Therapist, you will be an integral member of a multi-disciplinary health care team that provides rehabilitative care and skilled nursing to clients, affording them the opportunity to receive the medical care required to remain at home.
Occupational Therapist, OT Responsibilities:
- Perform outpatient therapy evaluations in order to develop a plan of care for patients in designated senior living communities or in a group home setting for adults with intellectual disabilities
- Professionally operate at top of licensure utilizing current research-based approaches to care in order to maximize functional independence for your clients
- Work closely with your home health therapy partners to appropriately transition care from home health to outpatient services in order to achieve client centered goals in their living environment, allowing the right care, at the right time for each client
- Educate the client, family and community caregivers on the care plans created for your clients in order to achieve a team approach to maximizing functional independence
- Perform evaluations, treatments and proper discharge planning for your clients utilizing outpatient EMR platform to deliver and document on your individualized treatment approaches to meet the needs of your clients
Occupational Therapist, OT Qualifications:
- A current Vermont Occupational Therapist license.
- Prior home care experience preferred, but not required
- A minimum of one year of recent work experience as an Occupational Therapist.
- Graduation from an occupational therapy program accredited jointly by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA, as indicated by school transcript or diploma.
- Ability to work independently and manage time effectively.
- Strong interpersonal skills, organizational skills, and problem solving skills.
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
