Get Access To All JobsTips for Finding Visa Sponsorship as a Nurse Practitioner Or Physician Assistant
Target cap-exempt employers first
Hospitals affiliated with universities and nonprofit research institutions can file H-1B petitions year-round, bypassing the lottery. These employers include academic medical centers and federally qualified health centers, giving you a sponsorship path that doesn't depend on luck.
Confirm your degree meets specialty occupation standards
USCIS requires a specific bachelor's degree or higher tied directly to the role. For NPs and PAs, a master's degree is now the standard entry-level credential, which strengthens your specialty occupation argument significantly compared to older associate-level training.
Get your state license sorted before applying
Most employers won't begin sponsorship paperwork until you hold an active state license. Requirements vary by state, so start the licensing process early. Some states have longer processing timelines, which can delay your start date and the entire H-1B petition.
Understand that TN visas don't apply to this role
Physician Assistants and Nurse Practitioners are not listed on the TN visa's qualifying occupations. Canadian and Mexican nationals in these roles must pursue H-1B sponsorship instead. Confirming this upfront prevents wasted applications and helps you plan your timeline accurately.
Ask employers about O-1 eligibility if you have specialization
NPs and PAs with published research, national certifications, or clinical leadership experience may qualify for an O-1A visa, which has no lottery or annual cap. This path is worth exploring if you have a strong professional record in a subspecialty area.
Prioritize employers with established immigration infrastructure
Large hospital systems and multi-site healthcare groups sponsor regularly and maintain relationships with immigration attorneys. These employers process petitions faster and make fewer errors than smaller practices attempting sponsorship for the first time, reducing your risk of delays or denials.

Find Nurse Practitioner Or Physician Assistant JobsFrequently Asked Questions
Which visa types are typically used to sponsor Nurse Practitioners and Physician Assistants?
The H-1B is the primary visa for NPs and PAs. Both roles qualify as specialty occupations given the master's-level education now required for licensure. Some employers, particularly academic medical centers and nonprofit hospitals, are cap-exempt and can file H-1B petitions outside the annual lottery window, making them especially valuable targets.
Do NP and PA roles qualify for the H-1B specialty occupation standard?
Yes. USCIS has consistently approved H-1B petitions for Nurse Practitioners and Physician Assistants because both roles require at minimum a master's degree in a specific clinical field. The degree-to-job alignment is direct and well-documented, which reduces the risk of a Request for Evidence compared to roles where the educational requirement is more ambiguous.
How can I find healthcare employers who actively sponsor NPs and PAs?
Migrate Mate lists visa-sponsoring healthcare employers actively hiring NPs and PAs, filtered by sponsorship type. Searching there is more efficient than applying broadly, since employers on the platform have indicated willingness to sponsor. Prioritize large hospital systems, federally qualified health centers, and VA facilities, which have established immigration processes and sponsor consistently across multiple states.
Can international NPs or PAs use the EB-3 green card pathway?
Yes. Employer-sponsored EB-3 petitions are a common green card pathway for NPs and PAs, especially in underserved or rural areas where employers have stronger incentive to sponsor long-term. Some employers in Health Professional Shortage Areas may also support EB-2 National Interest Waiver applications, though self-petitioning under NIW requires demonstrating national significance beyond standard clinical work.
Does the H-1B lottery create significant risk for NPs and PAs seeking sponsorship?
It depends on your employer type. For cap-subject employers, the roughly 25% lottery selection rate is a real risk. However, many healthcare employers, including university hospitals, nonprofit health systems, and federally funded clinics, are cap-exempt. These organizations can file at any time of year, eliminating lottery risk entirely and making them a strategically important segment to target in your job search.
What is the prevailing wage requirement for sponsored Nurse Practitioner Or Physician Assistant jobs?
U.S. employers sponsoring a visa must pay at least the prevailing wage, which is what workers in the same role, area, and experience level typically earn. The Department of Labor sets this rate to make sure companies aren't hiring foreign workers simply because they'd accept lower pay than a U.S. worker. It varies by job title, location, and experience. You can look up current prevailing wage rates for any occupation and location using the OFLC Wage Search page.
