Verify your ABPMR eligibility before applying
Many PM&R fellowship and attending positions require ABPMR board eligibility or certification as a condition of employment, not just sponsorship. Confirm your exam pathway and timeline before targeting roles, since employers won't initiate an LCA for a candidate who can't meet licensure within the visa period.
Target VA hospitals and academic rehab programs
VA medical centers and university-affiliated inpatient rehabilitation units are among the most consistent PM&R sponsors because federal funding structures and academic missions make them cap-exempt H-1B filers, letting them sponsor year-round without lottery exposure.
Pull prevailing wage data before salary negotiations
Use OFLC Wage Search to look up the prevailing wage for your specific SOC code and the employer's county before any offer conversation. PM&R wages vary significantly between rural critical-access hospitals and urban academic centers, and the LCA must certify at least the DOL wage level.
Clarify PERM timing if you want a green card path
Ask prospective employers whether they support EB-2 or EB-3 PERM sponsorship alongside the H-1B. For PM&R physicians, some employers bundle the PERM process with the initial offer, while others require two to three years of employment before initiating it.
Use Migrate Mate to file your H-1B petition cleanly
Once your employer signs off on the LCA, use Migrate Mate to handle the H-1B petition filing. A dedicated visa expert manages the I-129 paperwork and specialty occupation documentation specific to PM&R roles, reducing back-and-forth with USCIS on requests for evidence.
Negotiate premium processing into your offer letter
USCIS premium processing upgrades adjudication to 15 business days and is particularly valuable for PM&R physicians starting a new hospital rotation schedule or covering a position with a fixed start date. Ask HR to include this commitment in writing before you sign.
Frequently Asked Questions
Which visa types do PM&R physicians most commonly use for U.S. sponsorship?
Most PM&R physicians enter on an H-1B, which requires a specialty occupation determination and a DOL-certified LCA. J-1 visa exchange visitor visas are common for residency and fellowship but carry a two-year home residency requirement unless waived. Some academic and VA positions also sponsor O-1 visas for physicians with exceptional research or clinical distinction. The H-1B is the most common long-term employment pathway.
Does a PM&R residency completed outside the U.S. qualify for H-1B sponsorship?
It depends on credential equivalency. USCIS requires that foreign medical degrees be evaluated for U.S. equivalency, typically through a credential evaluation service. You'll also need to pass USMLE Steps 1, 2, and 3 and obtain an ECFMG certificate before most U.S. employers will sponsor you. Some states also require additional licensing steps that affect the LCA filing timeline.
Are PM&R physician roles cap-exempt for H-1B purposes?
The role itself isn't cap-exempt, but the employer can be. Positions at nonprofit hospitals affiliated with universities, VA medical centers, and certain research institutions qualify as cap-exempt H-1B employers, meaning they can file outside the annual lottery. If you're targeting cap-subject employers, you'll need to enter the H-1B registration lottery in March for an October 1 start date.
How do I find PM&R employers with active H-1B sponsorship history?
DOL's OFLC disclosure data shows which employers have filed LCAs for PM&R and related physiatry roles in recent fiscal years. You can filter by SOC code to identify hospitals and rehabilitation networks that have sponsored similar positions. Migrate Mate surfaces this employer data alongside active job listings, so you can focus your search on organizations with a demonstrated sponsorship track record.
Can I work as a PM&R physician on OPT before transitioning to an H-1B?
Yes, if you completed a U.S. medical degree or fellowship at an accredited institution, you may have OPT authorization. However, PM&R residency programs are graduate medical education, and OPT eligibility depends on your specific degree program structure. Most physicians in this specialty move directly from J-1 or H-1B status rather than OPT. Confirm your authorization type with your program's DSO before applying to attending positions.
What is the prevailing wage requirement for sponsored Physical Medicine and Rehabilitation Physician jobs?
U.S. employers sponsoring a visa must pay at least the prevailing wage, which is what workers in the same role, area, and experience level typically earn. The Department of Labor sets this rate to make sure companies aren't hiring foreign workers simply because they'd accept lower pay than a U.S. worker. It varies by job title, location, and experience. You can look up current prevailing wage rates for any occupation and location using the OFLC Wage Search page.
