Summary: Under the supervision of the Manager, the Social Work Assistant will provide support to Crisis Intervention licensed clinical social workers. This will allow the LCSW more time to complete vital clinical work both in the Emergency Department and Community (as part of Mobile Outreach). In addition, this will provide the patient, their family and/or case workers with more social work time. The new position will provide enhancements in care coordination and help avoid rapid re-admissions.
Responsibilities
- Assists in care coordination - discharge plans, referrals and transfers.
- Participate as second responder in mobile outreaches.
- Clerical work and paperwork necessary for patient's treatment.
- Provides a safe and/or secure environment for the patient/community.
- Fulfills all compliance responsibilities related to the position.
- Performs other duties as assigned.

BACHELOR'S LVL DGRE

Required: Excellent communication skills, works well taking direction, team player and flexible.
Minimum Experience

Experience working with persons with psychiatric/emotionally distressed or substance abuse population. American Heart Association Basic Life Support, CPR and AED Program.
