Job Summary

As a Behavioral Health Case Manager / Social Worker, you’ll coordinate compassionate, practical care for people navigating behavioral health needs. You’ll assess patients, support treatment and discharge planning, and connect individuals and families with appropriate services. Your impact: By coordinating care and community resources, you help patients and families move toward safer, more supported transitions.

Duties

• Complete intake, patient assessment, and diagnostic evaluation; document findings in electronic medical records (EMR/EHR) while following Health Insurance Portability and Accountability Act (HIPAA) requirements.
• Provide mental health counseling and addiction counseling, and coordinate clinical case management for individuals with mental health conditions, substance use disorders, and disabilities.
• Develop care and discharge plans; coordinate community resources and collaborate with patients, families, and care teams.
• Support utilization management and patient service across inpatient, acute care, hospital medicine, emergency medicine, intensive care unit (ICU), pediatric intensive care unit (PICU), operating room (OR), outpatient, clinic, and post-acute care settings, as applicable.
• Maintain accurate medical records and electronic health records (EHR); prioritize tasks and communicate clearly with clients and colleagues.

Requirements

• Background in social work, behavioral health, case management, patient care, and working with families and people from diverse cultural backgrounds.
• Knowledge of medical terminology and physiology; skills in patient assessment, client communication, and EHR management.
• Basic life support (BLS) and cardiopulmonary resuscitation (CPR) credentials as required for the role.
• Relevant experience may include hospitals, Level I or II trauma centers, acute care, ICU, PICU, OR, hospital or emergency medicine, pediatrics, inpatient, outpatient, clinics, managed care, or post-acute care.
• Experience with children and individuals with substance use disorders or mental health conditions is valuable.

Pay: $15.00 - $23.00 per hour

Expected hours: No less than 40.0 per week
