As a Clinical Case Manager, you'll play a vital role in helping clients navigate every step of their treatment journey while preparing them for long-term success. You'll advocate for your clients, coordinate essential resources, support treatment planning, and ensure every individual leaves with a personalized plan for continued recovery.

What You'll Do

- Coordinate admissions, assessments, treatment planning, and discharge planning to ensure seamless continuity of care.
- Connect clients with community resources, medical providers, aftercare services, legal resources, and other support systems that promote long-term recovery.
- Collaborate closely with therapists, physicians, nurses, Behavioral Health Technicians (BHTs), admissions, utilization review, and families to deliver exceptional client-centered care.
- Facilitate psychoeducational and process groups, as assigned.
- Assist with crisis intervention, behavioral support, and client advocacy while maintaining a safe, structured therapeutic environment.
- Complete timely, accurate documentation while ensuring compliance with regulatory, accreditation, and organizational standards.
- Coordinate warm handoffs to outpatient providers, recovery resources, and continuing care services that support lasting success beyond treatment.
We're looking for someone who is:
- Passionate about helping individuals recover from substance use and co-occurring mental health disorders.
- Highly organized and able to balance multiple priorities in a fast-paced clinical environment.
- A collaborative team player who enjoys working alongside therapists, nurses, physicians, and other behavioral health professionals.
- Skilled at building relationships with clients, families, referral partners, and community resources.
- Compassionate, resourceful, and committed to helping clients overcome barriers to recovery.
- An excellent communicator who values professionalism, accountability, and teamwork.
Qualifications
- Bachelor's degree in Social Work, Psychology, Human Services, Behavioral Health, Counseling, or a related field required.
- Experience in behavioral health, case management, substance use disorder treatment, or a related clinical setting preferred.
- Knowledge of treatment planning, discharge planning, case management, community resources, and continuity of care.
- Familiarity with trauma-informed care, ASAM Criteria, HIPAA, 42 CFR Part 2, and behavioral health best practices.

How do I get a case manager job in Denver?
Focus your search on Denver Health, Denver Human Services, and the network of behavioral health nonprofits anchored in Capitol Hill and Five Points. Candidates with experience in Medicaid-funded programs, housing-first initiatives, or substance use recovery services have a clear edge in this market. Bilingual Spanish fluency is a genuine differentiator given Denver's client population, and familiarity with Colorado's CCMS case management software strengthens applications to county-funded roles.
Which companies hire case managers in Denver?
Employers hiring case managers in Denver right now include COLORADO COALITION FOR THE HOMELESS, URBAN PEAK, and HCA HealthONE Presbyterian St. Luke's, based on current listings on Migrate Mate as of August 2026. Denver's hiring mix is anchored by large integrated health systems, county human services departments, and a dense cluster of community mental health centers serving the metro area.
Are there remote case manager jobs in Denver?
Yes, though availability depends heavily on role type. Field-based and crisis-intervention case managers work on-site by necessity, while utilization management and insurance-side care coordination roles are far more remote-friendly. About 21% of case manager openings tied to Denver are remote or hybrid as of August 2026. Payer-side and managed care roles originating from Denver's insurance sector offer the most consistent remote flexibility.
How can I get a case manager job in Denver with little or no experience?
The most realistic entry path in Denver is through a case manager aide or care coordinator role at a community health center or county-contracted nonprofit, where on-the-job training is built into the position. Denver Human Services and Arapahoe County Human Services regularly hire entry-level eligibility technicians who move into case management after six to twelve months. Volunteering with Denver's shelter and housing assistance network also builds the client-facing documentation experience local employers look for.
