Position Summary
Hamilton-Madison House’s Supported Housing program provides long-term housing options for individuals with Serious and Persistent Mental Illness (SPMI), enabling them to live independently within the community. The Case Manager supports tenants in maintaining stable housing, improving independent living skills, and accessing community-based resources. This role emphasizes recovery-oriented, trauma-informed care and adherence to OMH guidelines and standards.
Responsibilities
- Provide ongoing case management services to a caseload of tenants in supportive housing
- Conduct initial and ongoing assessments of tenant needs, strengths, and goals
- Develop, implement, and regularly update individualized service plans in collaboration with tenants
- Assist tenants in obtaining and maintaining benefits (e.g., SSI/SSD, SNAP)
- Support linkage to mental health, medical, and substance use treatment providers
- Conduct regular home visits to monitor housing stability, safety, and well-being
- Provide crisis intervention and de-escalation as needed
- Advocate on behalf of tenants with landlords, service providers, and community agencies
- Maintain accurate, timely, and compliant documentation in accordance with OMH and agency standards
- Participate in team meetings, case conferences, and supervision
- Facilitate life skills training, including budgeting, and daily living skills
- Promote community integration and social engagement
Minimum Qualifications
- Bilingual proficiency required in English and Korean, Cantonese, or Mandarin
- Bachelor’s degree in Social Work, Counseling/ Psychology or related field preferred
- Experience supporting individuals with serious mental illness, particularly within supportive housing settings
- Strong organizational, documentation, and communication skills
- Ability to work both independently and collaboratively within a multidisciplinary team
Pay Range: $26.50 to $28.00 per hour, commensurate with qualifications and experience
Hours: Monday to Friday, 9:00AM to 5:00PM, 37.5 hours per week
