Job Summary

The Social Worker is responsible for providing coordinated care in the home setting for hospice patients. This role conducts comprehensive psychosocial assessments, provides counseling and emotional support, identifies patient and family needs, and facilitates referrals to appropriate community resources. The Social Worker works collaboratively with the interdisciplinary team to ensure holistic care that supports the emotional, social, financial, and environmental well-being of patients and their families.

Essential Responsibilities
Conduct comprehensive psychosocial assessments to identify patients' and families' emotional, social, financial, environmental, and community resource needs.
Provide counseling, support, advocacy, and resource referrals based on assessed needs and the established plan of care.
Collaborate with the interdisciplinary team to develop, implement, and evaluate individualized patient care plans.
Attend and actively participate in interdisciplinary team meetings and patient care conferences.
Utilize effective communication and interpersonal skills to promote positive outcomes for patients, families, and team members.
Maintain compliance with all mandatory continuing education requirements and applicable licensing standards.
Demonstrate ongoing professional growth by maintaining current knowledge of public and private assistance programs, eligibility requirements, and community resources.
Promote and uphold the agency's mission, values, philosophy, and policies to ensure the highest quality of care.
Accept and perform clinical assignments consistent with education, licensure, training, and demonstrated competency.

Job Qualifications
Education

Master's Degree in Social Work (MSW) from an accredited program approved by the state.

Licensure
Current Texas Social Work licensure/certification in good standing, as required by state regulations.
Valid Texas Driver's License.
Experience
Minimum of one (1) year of social work experience in a healthcare setting, preferably in hospice.
Skills
Working knowledge of community resources and support services available to patients and families.
Strong interpersonal, communication, and counseling skills.
Ability to establish and maintain effective working relationships with patients, families, healthcare professionals, and community partners.
Excellent organizational, critical thinking, and problem-solving abilities.
Ability to work independently while collaborating effectively within an interdisciplinary team environment.
Transportation
Reliable transportation for travel to patient homes and other assigned locations.
Current automobile liability insurance meeting state requirements.
