Social Work Coordinator – Skilled Nursing

We are seeking a compassionate, organized, and resident-focused Social Work Coordinator to join our Skilled Nursing team.
Position Summary
The Social Work Coordinator serves as an advocate and resource for our Skilled Nursing residents and their families. This position works closely with residents, families, nursing staff, physicians, therapists, administration, and other members of the interdisciplinary team to support residents’ psychosocial needs and ensure a smooth and positive experience throughout their stay.
The ideal candidate will be compassionate, approachable, highly organized, and comfortable communicating with residents and families during both routine and challenging situations.
Essential Responsibilities
Serve as an advocate for residents and help ensure their psychosocial, emotional, and social needs are identified and addressed.
Complete social services assessments and participate in the development and ongoing review of resident care plans.
Meet with residents and families to identify concerns, provide support, and assist with problem-solving.
Coordinate and participate in care conferences and interdisciplinary team meetings.
Assist residents and families with the transition to and from Skilled Nursing, including discharge planning and coordination of services.
Communicate with hospitals, physicians, community agencies, home health providers, and other outside organizations as needed.
Assist residents and families in understanding available resources, services, benefits, and community programs.
Respond to resident and family concerns and work collaboratively with the interdisciplinary team toward appropriate resolutions.
Maintain accurate and timely documentation in accordance with facility policies and applicable regulations.
Assist with advance directives, guardianship concerns, power of attorney documentation, and other psychosocial matters as appropriate.
Provide emotional support to residents and families during times of illness, transition, loss, and adjustment.
Support resident rights and promote dignity, choice, privacy, and quality of life.
Participate in quality improvement initiatives and other departmental or facility projects as assigned.
Maintain confidentiality and comply with all applicable HIPAA, federal, state, and facility requirements.
Perform other duties as assigned.
Qualifications
Bachelor’s degree in Social Work, Human Services, Psychology, Sociology, or a related field required; Master’s degree preferred.
Previous experience working with older adults, preferably in a Skilled Nursing, long-term care, rehabilitation, assisted living, or senior living environment.
Knowledge of psychosocial needs of older adults and their families.
Strong communication, interpersonal, and organizational skills.
Ability to work effectively as part of an interdisciplinary team.
Ability to handle sensitive situations with professionalism, empathy, and discretion.
Strong problem-solving and conflict-resolution skills.
Proficiency with electronic documentation and Microsoft Office.
Must be able to maintain accurate records and meet required deadlines.
Knowledge of applicable Skilled Nursing regulations and resident rights is preferred.
