The Social Services Specialist Responsibilities Include
- Being a member of a collaborative practice model that includes patients, nurses, hospital care coordinators, physicians, and other practitioners, caregivers, and the community
- The Social Service Specialist II provides therapeutic interventions through assessment, coordination, referral and interdisciplinary planning
- They are knowledgeable about treatment, social and economic implications for individuals and families as it pertains to their age, stage of development and discharge needs
- Possess a clear understanding of discharge and transition planning and linkage with community resources
- Demonstrates skills in planning, organizing, and managing multiple functions and complex processes
Position Details
- Location: St Josephs Hospital - Tampa, FL
- Status: Part time, 16 hours per week
- Schedule: Saturday & Sunday 8:00 AM - 4:30 PM
- Weekend requirement: Every
- On Call: No
Certifications And Licensures
- Preferred ACM (Case Management) or
- Preferred CCM (Case Manager)
Education
- Required Masters in Social Work, Sociology, or Related Field
- Strongly Preferred Masters Degree in Social Work
Experience
- Required 6 months Internship or
- Required 1 year Social Work in an acute care or hospital setting
