Job Summary And Responsibilities
The Cardiovascular Program Specialist is involved in the coordination and evaluation process from candidate referral, evaluation, acceptance, procedure and follow-up for Ventricular Assist Device (VAD) patients, Cardiovascular surgery and other CV specialty procedure patients. The incumbent serves as a resource to staff and the multidisciplinary team and community caring for the patient. Demonstrates knowledge and skills in the care of patients with end-stage disease and will play a role in the holistic evaluation of patients. Skills in the area of critical thinking, interpersonal communication, conflict management, problem solving, autonomy and decision-making are highly desirable. The CV Program Specialist is also responsible for quality performance improvement of the specific CV program. Responsibilities include performing chart reviews, timely data abstraction and submission, providing various reports and data interpretation for performance indicators required by The Joint Commission, CMS and other regulatory agencies. The CV Program Specialist, in collaboration with the multidisciplinary team, develops Policies and Procedures, order sets and clinical pathways to provide and improve overall quality of care and program development. On call nights and weekends required.
- Coordinates the referral process and introduction of patients for purposes of evaluation and assessment.
- Receives incoming referrals from physicians & hospital & responds quickly to facilitate rapid access to the system. Actions include:
- contact with referring providers;
- requests, receives & reviews of pertinent clinical records;
- coordination of record review with multi-disciplinary team members.
- Works corroboratively with program Medical Director, and multi-disciplinary team to schedule evaluation and procedure
- Triages referrals according to written criteria to assure those patients needs are met on a priority level
- Communicates back to referring physician &/or hospital the plans for acceptance into the program
- Educates the patient and family regarding the referral process, steps of evaluation, & keeps patient & family informed throughout the entire process
Required Job Requirements
- Bachelors Of Nursing or equivalent work experience and 5 years cardiovascular experience, upon hire
- Evidence in caring for acute & chronic heart & vascular patients
- Registered Nurse: AZ, upon hire
Preferred
- Previous experience with VAD, ECMO Cv Surgery
- Previous experience in cardiac intensive care, cath lab or CTOR
