Homeward Bound, a behavioral health agency in Dallas, is looking for a fully licensed LCDC for the outpatient Medication-Assisted Treatment (MAT) program. Individuals enrolled in this program are recovering from opioid addiction through the use of buprenorphine and can be involved in a variety of levels of care including substance use outpatient, mental health outpatient, and substance use residential treatment.
This position involves strong attention to detail, the ability to be self-directed, high motivation, trauma-informed care, and the capacity to have working knowledge of two electronic health records: CMBHS and Netsmart. If you like to solve problems, blend administrative and clinical work, and create a fast-paced environment, then this could be a good position for you!
This is a full-time, Monday through Friday position .
Duties for this position include, but are not limited to
- Complete all admission documentation including the screening, assessment, and treatment plan for every client enrolled in MAT services.
- Do the initial financial eligibility form and update it on enrolled clients every 6 months.
- Complete treatment plan reviews every 30 days.
- Have individual counseling sessions with clients on a regular basis.
- Complete discharge documentation after staffing concerns and problem solving with the MAT team for clients who complete, choose to discontinue, or who disengage with the program.
- Work closely with the medication prescribers and other medical professionals to handle the clinical concerns of each client.
- Be adept at understanding a variety of funding sources and how to coordinate individualized care utilizing those sources.
- Provide continuity of care by coordinating with other internal and external programs clients may be active with.
- Be available to assist with medication issues such as communication with the pharmacy, medication pick up, and medication discrepancies.
- Assist with urine drug screens and the documentation of urine drug screens as necessary.
- Assist with data collection for the MAT program which will include conducting surveys, attending MAT-related meetings, and keeping accurate records of all clients through CMBHS and MAT documentation as well as through organizational spreadsheets.
- Be able to explain the differences in Subutex, Suboxone, and Sublocade to both clients and staff from other departments.
