SUMMARY
AS A PHLEBOTOMIST, you will be responsible for performing and attaining phlebotomy samples of all types except arterial on in-patients and out-patients.
BASIC QUALIFICATIONS
- High school graduate or equivalent required
- Prior experience working in the medical setting
- Must have taken a certified phlebotomy course; must attain ASCP certification within 2 years of being hired to role (or completion of a practice nursing program)
- Ability to work independently

Does a phlebotomist job qualify for CPT authorization?
Yes, if your enrolled program includes a clinical practicum, externship, or lab rotation that specifically covers venipuncture or specimen collection. Your DSO will check whether the phlebotomist job description maps to that coursework. Roles at hospitals, reference laboratories, blood donation centers, and outpatient clinics typically qualify, but your DSO makes the final call based on your program's catalog language.
Can I work phlebotomy CPT at multiple sites simultaneously?
Only if your DSO authorizes each site separately and your I-20 reflects all authorized employers. Some programs allow concurrent CPT at two clinical sites when different rotations require different settings, such as a hospital inpatient draw station and an outpatient lab. Each site needs its own authorization. Working at an unauthorized site while on CPT is a status violation.
How do I find phlebotomy employers who are comfortable hiring F-1 CPT students?
Search on Migrate Mate, which shows employers with documented work-authorization experience so you're targeting sites already familiar with F-1 documentation requirements. Large health systems and national reference labs tend to have established onboarding processes for CPT students. Smaller independent labs may be willing but unfamiliar with the paperwork, so come prepared with a one-page CPT explainer from your DSO.
What happens to my CPT authorization if the clinical site closes or reassigns me?
Your CPT authorization is tied to the specific employer on your I-20. If you're reassigned to a different legal entity, a new address under a separate tax ID, or a partner facility not listed on your authorization, you need a new CPT authorization before you start work there. Contact your DSO immediately when any site change is proposed. Working without updated authorization is a status violation regardless of how minor the change seems.
Does full-time phlebotomy CPT affect my OPT eligibility later?
Full-time CPT used for 12 months or more cumulatively eliminates your OPT eligibility, so tracking your total full-time CPT hours matters. Part-time CPT, defined as 20 hours or fewer per week, does not affect OPT regardless of duration. If your phlebotomy practicum requires full-time hours over multiple semesters, plan your post-graduation authorization strategy with your DSO well before you reach the 12-month threshold.
