Job Summary
Compassionate and dedicated Behavioral Health and Social Services professional with a Master’s degree in Behavioral or Social Science and extensive experience in psychiatric outpatient care, behavioral health case management, and clinical support services. Skilled in conducting assessments, developing individualized treatment and service plans, crisis intervention, counseling, and coordinating comprehensive care for individuals with developmental disabilities, mental illness, and emotional disturbances. Experienced in collaborating with multidisciplinary treatment teams, managing client safety and risk, and connecting patients with community resources and social service agencies. Strong ability to utilize effective interviewing, counseling, and case management techniques to support patient independence, emotional well-being, and successful treatment outcomes while maintaining compliance with behavioral health standards and regulations.
Job Duties:
Performs professional social work in a psychiatric outpatient setting by overseeing and participating in assisting the developmental disabilities, mentally ill or emotionally disturbed to readjust and achieve a social, economic, emotional, and physical balance and personal independence compatible with generally accepted norms. Professional behavioral health case management services include but are not limited to assessments, treatment planning, direct service provision, person-centered services and management of client risk and safety. Reports to an administrative superior and is responsible for providing comprehensive patient care services that support the integrity of the Department of Health and Human Services – Division of Mental Health and Substance Abuse. Provides consultative services to other state and private agencies as well as referring clients/patients to appropriate social service agencies.
Minimum Qualifications
Possession of a master’s Degree in Behavioral or Social Science or related field.
Three years’ experience in case management which includes assessing, planning, developing, implementing, monitoring, and evaluating options and services to meet an individual’s human service needs using effective counseling or interviewing techniques as well as other available resources such as service plans or case management systems.
Three years’ experience in providing clinical treatment, counseling, or determining eligibility for services or benefits as part of a treatment team.
