H-1B Visa Clinical Therapist Jobs
Clinical Therapist roles qualify as H-1B specialty occupations because they require at minimum a master's degree in social work, counseling, or a related clinical field. Healthcare systems, community mental health centers, and private group practices all file H-1B petitions for licensed therapists. The annual cap applies, so timing your job search around the April lottery window matters.

Get Access To All JobsTips for Finding H-1B Visa Sponsorship in Clinical Therapist
Verify your licensure transfers across states
Your H-1B petition is tied to a specific employer and work location. If your clinical license is state-specific, confirm reciprocity or endorsement eligibility before accepting an offer, since USCIS requires you to be legally authorized to practice in that state.
Target employers with cap-exempt filing history
Hospitals affiliated with universities, federally qualified health centers, and nonprofit research institutions can file H-1B petitions outside the annual lottery cap. Focusing your search on these employer types removes the random selection risk entirely.
Pull LCA filings to assess real sponsorship patterns
Use Migrate Mate to search verified DOL Labor Condition Application data filtered by clinical therapist roles. You'll see which employers have actively sponsored therapists, not just which ones say they're open to it.
Confirm your role meets specialty occupation criteria
Check the O*NET profile for Clinical, Counseling, and School Psychologists or Mental Health Counselors to document that your specific position requires a directly related master's or doctoral degree. USCIS scrutinizes therapist petitions where job duties could be interpreted as requiring only a general bachelor's.
Negotiate an offer before the April registration window
H-1B cap registrations open in March and close within days. Secure your offer letter and have your employer retain an immigration attorney or filing service by late February so the I-129 petition and supporting documentation are ready the moment registration opens.
Request a prevailing wage determination before your offer is finalized
Your employer must pay at least the DOL prevailing wage for your occupation and work location, verified through the OFLC Wage Search. Ask for the wage level used in the LCA before signing, since Level I wages can be significantly below market for licensed clinicians.

Find Clinical Therapist JobsClinical Therapist H-1B Visa: Frequently Asked Questions
Does a Clinical Therapist role qualify as an H-1B specialty occupation?
Yes, provided the position requires at minimum a master's degree in a directly related field such as clinical social work, marriage and family therapy, or professional counseling. USCIS has issued requests for evidence on therapist petitions where job postings accepted any bachelor's degree, so the offer letter and job description must explicitly require a specific graduate-level credential tied to the clinical duties.
Which types of employers are most likely to sponsor H-1B visas for therapists?
Large hospital systems, university-affiliated behavioral health programs, federally qualified health centers, and nonprofit community mental health organizations file the highest volume of H-1B petitions for clinical roles. These employers often have established immigration processes and legal counsel in place. You can browse verified sponsoring employers filtered by clinical therapist LCA filings on Migrate Mate.
Can a private therapy practice or small group practice sponsor an H-1B?
Yes, but most small and solo practices lack the infrastructure to navigate the process independently. The employer must file a certified Labor Condition Application with DOL before submitting the I-129 petition to USCIS, post the LCA at the worksite, and pay prevailing wages. Small practices that do sponsor typically work with an outside immigration attorney to handle compliance requirements.
How does state licensure affect my H-1B petition as a therapist?
Your H-1B approval does not grant you clinical licensure. Each state licenses independently, and USCIS expects you to hold or be eligible for the license required to practice in your work location. If you're relocating from another state, confirm that your credential qualifies for endorsement or reciprocity before your employer files, since a licensure gap can complicate the petition or delay your start date.
What happens to my H-1B status if my employer changes billing structure or contract terms after I start?
Material changes to the terms of your employment, such as a shift from salaried to contract billing, a significant reduction in hours, or a change in work location, may require your employer to file an amended H-1B petition with USCIS. The prevailing wage obligation and LCA must reflect actual working conditions throughout your authorized period, not just at the time of the original filing.
