Get Access To All JobsTips for Finding H-1B Visa Sponsorship in Family Medicine
Verify your ECFMG certification before applying
ECFMG certification is a hard requirement for international medical graduates entering residency or fellowship in the U.S. Without it, no H-1B sponsor can legally employ you as a physician. Confirm your status is current before contacting any employer.
Target federally qualified health centers
FQHCs and rural health clinics consistently sponsor H-1B physicians because they operate in designated Health Professional Shortage Areas. These employers often have standing LCA filings for Family Medicine roles and are accustomed to the H-1B petition process.
Search H-1B filing history on Migrate Mate
Filter by Family Medicine SOC codes on Migrate Mate to see which employers have active LCA filings in your target region. This lets you focus applications on employers with a documented track record of sponsoring physicians, not just those who say they might.
Check prevailing wage before negotiating your offer
Your employer must pay at least the DOL prevailing wage for your role and location. Use OFLC Wage Search to look up the Level I through Level IV wage tiers for Family Medicine physicians in the specific metropolitan area before signing any offer letter.
Understand the J-1 waiver timeline if it applies
If you completed residency on a J-1 visa, you'll need a waiver of the two-year home residency requirement before USCIS can approve your H-1B. Conrad 30 and federal agency waivers each have separate application windows that must align with your employment start date.
Confirm cap-exempt status with academic medical centers
Hospitals affiliated with universities or research institutions may qualify as cap-exempt H-1B employers, meaning your petition doesn't compete in the annual lottery. Ask directly whether the sponsoring entity holds cap-exempt status under INA Section 214(c)(1).
H-1B Visa Family Medicine: Frequently Asked Questions
Do Family Medicine physicians qualify as a specialty occupation for H-1B purposes?
Yes. Family Medicine requires a Doctor of Medicine or Doctor of Osteopathic Medicine degree, followed by a three-year accredited residency. USCIS treats it as a specialty occupation because the role normally requires at least a bachelor's degree in a directly related field as a minimum entry requirement, and a medical degree far exceeds that threshold.
How do I find Family Medicine employers who actively sponsor H-1B visas?
Search by Family Medicine SOC codes on Migrate Mate to surface employers with verified LCA filing history. Hospitals in Health Professional Shortage Areas, federally qualified health centers, and academic medical systems file the most petitions for this role. Filtering by location and filing volume narrows the list to employers already familiar with the process.
Can I start working while my H-1B petition is pending after residency?
If you're transitioning from a J-1 visa training visa or an OPT-authorized F-1 status, cap-gap provisions may not apply the same way they do for F-1 holders. Your ability to work during the gap depends entirely on your current status and its expiration date. Confirm the exact bridge authorization with your sponsoring employer's immigration counsel before your current status expires.
What does the LCA process look like for a Family Medicine role?
Your employer files a Labor Condition Application with the DOL before submitting the I-129 petition to USCIS. The LCA certifies that the offered wage meets the prevailing wage for your role and work location, and that hiring you won't adversely affect similarly employed U.S. workers. DOL typically certifies LCAs within seven business days when filed through FLAG.
Does completing a U.S. residency affect my H-1B eligibility or cap count?
Completing a U.S. residency program on a J-1 or H-1B visa counts against your six-year H-1B maximum stay if those years were spent in H-1B status. J-1 residency time doesn't reduce your H-1B clock, but triggers the two-year home residency requirement unless waived. Check your I-94 history and visa type for each training year to calculate your remaining H-1B time accurately.
