H-1B Visa Family Physician Jobs
Family Physician roles qualify as H-1B specialty occupations because they require a medical degree plus residency training. Many hospitals, federally qualified health centers, and private practices sponsor H-1B petitions for primary care physicians, particularly in underserved areas where demand consistently outpaces the domestic supply.

Get Access To All JobsTips for Finding H-1B Visa Sponsorship as a Family Physician
Verify your ECFMG certification before applying
International medical graduates need ECFMG certification before a U.S. employer can file your H-1B petition. Confirm your certification status is current and your USMLE steps are complete so you don't stall an offer at the paperwork stage.
Target federally qualified health centers specifically
FQHCs and rural health clinics frequently sponsor H-1B physicians and often qualify as cap-exempt employers through their HRSA affiliations. This means you can file outside the annual lottery window, giving you a timeline advantage over private-sector candidates.
Search H-1B filing history on Migrate Mate
Filter Family Physician roles by employers with verified H-1B LCA filing history on Migrate Mate. Seeing actual DOL Labor Condition Application filings by occupation code tells you which employers have sponsored physicians before, not just which ones claim to.
Confirm the employer files before your J-1 waiver deadline
If you're on a J-1 Conrad 30 waiver, your H-1B petition must be filed while your waiver status is valid. Confirm upfront that your prospective employer has filed H-1B petitions in recent cycles and can meet the USCIS premium processing timeline if needed.
Run the OFLC Wage Search for your county and specialty
Your employer's LCA must certify a wage at or above the DOL prevailing wage for your specific SOC code and work location. Pull the OFLC Wage Search for your county before negotiating your offer so you know the floor your employer is legally required to meet.
Ask whether the position is licensed at state level
Your H-1B approval depends partly on holding a valid state medical license for the practice location. Some employers sponsor before licensure is finalized using a restricted license, but USCIS requires the full license before you can begin patient care under H-1B status.

Find Family Physician JobsFamily Physician H-1B Visa: Frequently Asked Questions
Does a Family Physician role qualify as an H-1B specialty occupation?
Yes. Family Physician positions require a Doctor of Medicine or Doctor of Osteopathic Medicine degree plus completion of an accredited residency program, which satisfies USCIS's specialty occupation standard. The O*NET occupation profile for Family Medicine Physicians confirms a doctoral-level education requirement, making this one of the cleaner specialty occupation determinations USCIS adjudicators encounter.
Which types of employers are most likely to sponsor H-1B visas for Family Physicians?
Federally qualified health centers, rural health clinics, Veterans Affairs facilities, and academic medical centers are the most consistent H-1B sponsors for Family Physicians. Many qualify as cap-exempt employers, meaning they can file H-1B petitions year-round without competing in the annual lottery. You can identify employers with verified filing history by browsing Family Physician roles on Migrate Mate.
Can I transfer my H-1B to a new Family Physician employer mid-year?
Yes. H-1B portability under AC21 lets you start working for a new employer as soon as the new petition is filed with USCIS, without waiting for approval, as long as your previous petition was approved and you've been in valid H-1B status. Your new employer files a new I-129, and your prior approval period doesn't carry over, so confirm the new employer can match or exceed your remaining authorized stay.
How does the J-1 Conrad 30 waiver interact with H-1B sponsorship for physicians?
Conrad 30 waiver recipients must work in a designated shortage area or health professional shortage area for three years. During that period, your employer can file an H-1B change of status on your behalf. The H-1B petition must be filed before your J-1 and waiver status expire. USCIS allows concurrent J-1 waiver service and H-1B approval as long as filings are properly sequenced.
What happens to my H-1B status if my state medical license lapses?
H-1B status for physicians is conditioned on maintaining a valid state medical license for the work location. If your license lapses or is placed on restricted status, you can't legally practice under your H-1B, and USCIS could consider your status violated. Employers are required to notify USCIS of material changes in your position, which a license lapse can trigger. Renew your license well before expiration to avoid any gap.
