Get Access To All JobsTips for Finding H-1B Visa Sponsorship in Internist
Verify your ECFMG certification before applying
International medical graduates need active ECFMG certification before any U.S. employer can file an H-1B petition. Confirm your certification status is current and matches the name on your passport to avoid processing delays at USCIS.
Target teaching hospitals and large health systems
Academic medical centers and multi-site health systems file H-1B petitions regularly for internists and have established immigration teams. Community practices often lack that infrastructure, so focus your search on institutions with documented sponsorship history.
Search verified sponsoring employers on Migrate Mate
Filter internist roles by employers with confirmed H-1B LCA filing history using Migrate Mate. This surfaces health systems that have actively sponsored the role before, so you're not guessing at sponsorship willingness during outreach.
Understand the prevailing wage before negotiating
Your employer's LCA must certify a wage at or above the DOL prevailing wage for your specialty and location. Look up the applicable wage level using the OFLC Wage Search before entering salary discussions so you know the federal floor.
Clarify J-1 waiver status early in the process
If you completed a J-1 residency, confirm whether your two-year home residency requirement has been waived before an employer files your H-1B. An unresolved J-1 obligation blocks USCIS from approving the petition regardless of employer sponsorship.
Request premium processing when your start date is fixed
Hospital credentialing calendars and residency program end dates create hard start deadlines. Ask your employer to file Form I-907 for premium processing so USCIS adjudicates the petition within 15 business days rather than several months.
H-1B Visa Internist: Frequently Asked Questions
Does an Internist role qualify as a specialty occupation for H-1B purposes?
Yes. Internal medicine requires a Doctor of Medicine or Doctor of Osteopathic Medicine degree plus completion of an accredited residency program, meeting USCIS's specialty occupation standard of a minimum bachelor's-equivalent degree in a specific field. The O*NET occupation profile for physicians confirms the advanced degree requirement, and USCIS consistently approves H-1B petitions for internists when the employer documents the degree requirements properly.
Which types of employers sponsor H-1B visas for internists?
Teaching hospitals, academic medical centers, large multispecialty groups, and federally qualified health centers are the most common H-1B sponsors for internists. These institutions file LCAs with DOL routinely and have immigration counsel on retainer. Smaller private practices occasionally sponsor but often lack the administrative capacity, making larger systems a more reliable target. You can filter for employers with verified LCA filing history on Migrate Mate.
How does the H-1B cap lottery affect physicians applying for internist jobs?
Most hospital employers and academic medical centers are cap-exempt under USCIS rules because they qualify as institutions of higher education or nonprofit research organizations affiliated with one. Cap-exempt filing means your employer can submit the H-1B petition at any time of year without entering the lottery, which significantly reduces timeline uncertainty for credentialing and start-date planning.
Can I switch from a J-1 visa to H-1B while working as an internist?
You can convert from J-1 to H-1B, but only after your two-year home residency requirement is either fulfilled or formally waived. Conrad 30 and federal agency waivers are the most common pathways for physicians. Once a waiver is approved, your employer files the H-1B change of status petition with USCIS. Starting the waiver process early matters because DOL certification and USCIS adjudication together can take six months or more.
What documents should I prepare before an employer files my H-1B petition?
You'll need your medical degree transcripts and diploma, ECFMG certification letter for international medical graduates, residency completion certificate, current state medical license or documentation of pending licensure, and a copy of your passport. Your employer's immigration attorney will also need your CV and a signed employment offer letter. Having all credentials authenticated and organized before the filing window shortens the process considerably.
