Verify your ECFMG certification before applying
International medical graduates must hold valid ECFMG certification before any U.S. employer can file an H-1B petition. Confirm your certificate is current and that your medical degree is listed correctly, since discrepancies create RFE delays after filing.
Target academic medical centers with fellowship programs
Teaching hospitals that run accredited interventional cardiology fellowships file H-1B petitions routinely for graduating fellows and recruited faculty. These institutions already have immigration counsel on retainer, which shortens the employer's internal approval timeline considerably.
Check the prevailing wage before negotiating your offer
Your employer's LCA must certify a wage at or above the DOL prevailing wage for your occupation and work location. Run your job title and metropolitan area through the OFLC Wage Search before salary discussions so you know the floor the employer is legally required to meet.
Use Migrate Mate to filter employers with H-1B filing history
Not every hospital that recruits internationally will sponsor H-1B visas. Migrate Mate surfaces verified DOL Labor Condition Application data by employer and specialty, so you can focus your applications on institutions that have actively filed for interventional cardiologist roles.
Request premium processing if your start date is fixed
Cath lab positions often have a firm credentialing and start-date timeline tied to hospital privileging cycles. If your offer letter specifies a start date, ask the employer to file USCIS Form I-907 for premium processing, which returns a decision within 15 business days.
Confirm J-1 waiver status before the employer files
If you completed your fellowship on a J-1 visa, you likely have a two-year home-residency requirement that blocks H-1B status until waived. Resolve the waiver through Conrad 30, a federal agency sponsorship, or USCIS before your employer submits the H-1B petition.
H-1B Visa Interventional Cardiologist: Frequently Asked Questions
Does an interventional cardiologist role qualify as a specialty occupation for H-1B purposes?
Yes. Interventional cardiology requires a medical degree followed by residency and fellowship training, which satisfies USCIS's specialty occupation standard of a minimum bachelor's degree in a directly related specific field. In practice, USCIS treats licensed physician roles as among the clearest qualifying specialties, and RFEs on specialty occupation grounds are uncommon for this title.
Which employers sponsor H-1B visas for interventional cardiologists?
Academic medical centers, large hospital systems, and multispecialty physician groups are the most consistent sponsors. Community hospitals sponsor less frequently but do file when recruiting for hard-to-fill cath lab positions. Migrate Mate shows verified LCA filing history by employer and occupation code, so you can identify institutions that have filed for interventional cardiologist roles specifically, rather than guessing from job postings.
Can an interventional cardiologist be hired at a cap-exempt institution to skip the lottery?
Yes. Nonprofit research organizations, universities, and certain government research entities are cap-exempt under the H-1B statute. If a medical school, VA hospital, or affiliated research institution employs you directly and you spend a portion of your time on qualifying research or teaching, the petition may be filed outside the annual cap and lottery entirely. Confirm the employer's cap-exempt status with their immigration counsel before accepting the offer.
How does the H-1B cap-gap rule affect an interventional cardiologist completing a fellowship on F-1 OPT?
If your OPT authorization expires between April 1 and September 30 and your employer filed an H-1B petition by April 1, the cap-gap rule automatically extends your OPT and work authorization through September 30 while the petition is pending. You can continue your fellowship without a gap in status. USCIS issues an updated I-94 reflecting the extension after confirming receipt of the petition.
What happens to H-1B status if an interventional cardiologist changes hospital systems?
H-1B status is employer-specific, so moving to a new hospital requires the new employer to file a fresh H-1B petition before your start date. Under AC21 portability, if you've held H-1B status for at least 180 days and are in the same or a similar occupation, you can begin working for the new employer once the petition is filed and received by USCIS, without waiting for approval.
