About This Hospice Response Team Registered Nurse (RN) Position
HopeHealth is seeking a Hospice Field RN for its affiliate HopeHealth Hospice & Palliative Care.
The Response Team RN will respond to urgent patient needs and require excellent clinical assessment skills and an ability to quickly identify the best course of action to treat patients. This is not a Case Management position and requires an RN who is comfortable in an action-oriented role who is comfortable assessing and treating a patient, then moving on to the next need.
This is a full-time day position working 3-11:30pm working every other weekend. This position is eligible for benefits
This position is eligible for the 3000 sign on bonus
What will you do as a Hospice Response Team Registered Nurse (RN)?
- Initial and ongoing assessment of the impact of the terminal diagnosis on the patient's physical, functional, psychosocial and environmental needs and activities of daily living.
- Implements the individualized plan of care and recommending revisions to the plan as necessary and plans for discharge/transfer as appropriate.
- Consults with and educates the patient/family/caregiver regarding the disease process and teaches the patient/family/caregiver self-care techniques, as appropriate. Provides medication, diet and other instructions as ordered by the physician.
- Initiates appropriate preventive and rehabilitative nursing procedures.
- Collaborate in the coordination of all patient/family services and prioritization of needs with the members of the interdisciplinary team to develop and revise patient's individualized plan of care.
- Informs the physician and other personnel of changes in the patient's needs and outcomes of intervention, updating Plan of Care as needed.
- Collaborate in the coordination to determines scope and frequency of services needed based on acuity and patient/family needs.
- Coordinates care with other interdisciplinary team members in cooperation with the family/caregiver to meet identified needs of the patient/family/caregiver; including psychosocial, spiritual, and emotional needs and refers to appropriate disciplines/services.

- Graduate of an accredited nursing school; RN required; BSN preferred
- Licensed as a Registered Nurse in Rhode Island and Massachusetts
- Minimum of 1-3 years of clinical experience
- Excellent assessment, clinical and communication skills
- Bilingual Preferred but not required
