ROLE AND RESPONSIBILITIES
As the Per Diem Registered Nurse Case Manager, you will assume full nursing responsibility for the delivery of the Plan of Care for each hospice patient. Continuously evaluate personal and professional performance and makes necessary changes to increase productivity and quality of care delivered. Adheres to and supports the policies/procedures/goals/objectives of hospice to assure quality patient care.
Primary Responsibilities:
- Makes the initial nursing evaluation in determining eligibility for hospice services during visit within forty-eight (48) hours of referral
- Identifies the patient's/family's physical, psychosocial, and environmental needs and re-assesses as needed, no less than every fifteen (15) days
- Initiates and coordinates the plan of care while maintaining accurate and relevant clinical notes regarding the patient's condition
- Documents problems, appropriate goals, interventions, and patient/family response to hospice care
- Collaborates with the patient/family, attending physician and other members of the IDG in providing patient and family care daily

BASIC QUALIFICATIONS
- Current unrestricted RN licensure in the state of practice
- 1+ years of clinical experience
- Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation
State Specific Requirements:
- LA: a minimum two years full time experience working as an RN. However, two years of full-time clinical experience in hospice care as a licensed practical nurse may be substituted for the required two years of experience as a registered nurse
PREFERRED QUALIFICATIONS
- Current CPR Certification or ability to complete within 90 days of hire
COMPENSATION
- Hourly pay for this role will range from $37.00 to $49.00 per hour based on full-time employment. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The company complies with all minimum wage laws as applicable.
