Responsibilities:
- Assess the psychosocial functioning of Palliative and Home Health care patients and families in their home environment and develop an individualized plan of care
- Provide appropriate psychosocial interventions in accordance with the physician's orders and the policies and procedures of the Agency
- Educate patient/family on, and assist in, preparation of advance directives
- Provides information and referral to patients and families/caregivers regarding necessary community resources
- Maintains clinical records on all patients referred to social work prepares clinical and progress notes
- Function as an integral member of the interdisciplinary team
Qualifications:
- Master of Social Work Degree
- New Grads welcome, we will train the candidate; clinical supervision is available
- Bilingual in Spanish and English
- People-oriented self-starter; notable written and verbal communication skills, and customer service skills
- California licensed and insured driver with reliable transportation
