Trustbridge Hospice, a proud member of Empath Health, is seeking a caring and motivated Hospice Registered Nurse (RN) Case Manager to join our dedicated hospice team in the Palm Beach County area. In this vital role, you will provide exceptional, patient-centered care while guiding families through one of life’s most meaningful journeys on the following schedule: Monday - Friday 8:30am - 5:00pm with a weekend requirement.

What you'll Do
- Serve as a key advocate for patients and families, ensuring their needs are recognized and addressed
- Coordinate comprehensive care under the direction of the primary physician and in collaboration with the interdisciplinary team
- Develop and implement individualized plans of care as part of the IDG (Interdisciplinary Group)
- Use your clinical expertise to monitor patient conditions, provide interventions, and deliver education that enhances comfort and quality of life
- Address concerns promptly and thoughtfully to ensure a high level of satisfaction and trust
What You'll Need
- Registered Nurse currently licensed in the State of Florida.
- Current Basic Life Support Certification.
- One year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing preferred.
- Understanding of and ability to apply characteristics of Hospice Nursing: case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs
- Basic computer skills and EMR software experience a plus.
- Adequate and reliable means of transportation with adequate insurance and valid Florida driver’s license.
