Job Summary

As a Hospice Registered Nurse, you’ll deliver compassionate, individualized care to patients and support their families through each stage of hospice care. You’ll assess patient needs, coordinate services, and communicate clearly with patients, caregivers, and healthcare professionals. Responsibilities

• Assess patients, monitor vital signs, evaluate symptoms, and apply nursing and physiology knowledge to guide care.
• Develop and update care plans; coordinate intake, clinical case management, discharge planning, and referrals across hospice, home health, and post-acute care.
• Collaborate with healthcare professionals to support patient care, service coordination, and continuity across care settings.
• Maintain accurate medical records and electronic health records (EHR), follow privacy requirements under the Health Insurance Portability and Accountability Act (HIPAA), and apply relevant health regulation policy.
• Communicate with patients and caregivers using clear, respectful language; document care and support patient service needs.

Requirements

• Active registered nurse license and nursing experience; hospice, home health, geriatrics, or post-acute care experience is preferred.
• Patient assessment, case management, utilization management, managed care, diagnostic evaluation, and electronic medical record (EMR) or EHR skills are valuable.
• Knowledge of medical terminology, infectious disease care, and care coordination; experience with hospital medicine, primary care, clinic, inpatient, acute care, medical-surgical, or emergency settings is helpful.
• Critical care, intensive care unit (ICU), pediatric ICU (PICU), pediatrics, emergency room, emergency medicine, operating room (OR), Level I or II trauma center, behavioral health, or hospital experience is also relevant.
• Experience managing patients as a nurse case manager, along with client communication and discharge planning, is preferred.

Pay: From $45.00 per hour

Expected hours: 40.0 per week
