LaSante Health Center is seeking an Internal Medicine Provider to join our multispecialty, community-based health center in Brooklyn. The ideal candidate will be committed to delivering comprehensive primary and preventive care to adult patients, managing chronic conditions, and promoting overall wellness in a culturally diverse community. Responsibilities:
Provide primary and preventive care for adult patients, including physical exams, screenings, and chronic disease management
Diagnose and treat acute and chronic medical conditions

Counsel patients on lifestyle modification, disease prevention, and medication management
Maintain accurate, timely documentation in the electronic health record (EHR)
