Summary
Provider cares for patients, by providing outpatient, primary care services that focus on patient centric measures such as access to care, value aligned prescribing, diabetes care, cardiovascular care. Provider will lead and work collaboratively with a team, including nurses, medical assistants, case and care managers, behavioral health clinicians, pharmacist clinicians, and clerical staff to provide care to patients. The position requires regular communication and collaboration with others. Panel size will be kept small to ensure good access, reduced costs and reduced need for the Emergency Department. Candidates will need to be versed in Value aligned care.

Presbyterian Medical Group seeks a Board Eligible / Board Certified Family Medicine or Internal Medicine trained physician to join our Medicare Advantage Clinic in New Mexico. Practice Highlights:
- Provide comprehensive medical care to patients with acute and chronic illnesses through in-person, telehealth, and asynchronous encounters
- Collaborate with Social Worker, Community Health, PHP benefits, pharmacy
- Orchestrate treatments, medications, and non-pharmacologic therapy plans in partnership with patient and their caregivers/family
- Promote population health management programs and services to patients
- 14-16 patients per day
Responsibilities:
- Provide comprehensive medical care to patients with acute and chronic illnesses with a focus on access, cost management, and reduced ED and hospital utilization
- Make professional medical judgments in the care of patients seen
- Order, conduct, and interpret appropriate diagnostic x-ray and lab tests
- Prescribe treatments, medications, and non-pharmacologic therapies with a strong alignment toward value
- Document all patient encounters and communications appropriately
- Manage the provider/patient relationship
- Receive and resolve patient complaints
- Involved in education and counseling of individuals, families, and groups
- Serve as a health care resources, interdisciplinary consultant, and patient advocate
- Work autonomously as well as in collaboration with other health care providers and staff as needed
- Serve as a model site for the growth and development of value aligned care
- Participate in the recruitment and replacement of other providers
- Participate in system-wide health management initiatives
- Provide input into the practice sites financial, clinical, and service quality performance; budget, capital plan, and personnel issues
- Provides clinical input into the development and review of appropriate clinical protocols and processes
- Enhance professional growth and development through participation in educational programs, literature review, in-services, and workshops
- Perform other functions as required
- Attend and participate in all scheduled meetings including PCC, provider and site meetings
- Modifies assessment techniques according to patients condition, culture, age, and stage of development
- Maintain coding and encounter data standards for completeness, accuracy, timeliness and reimbursement level, including legible documentation
